摘要
通过对居民全周期慢病防治的研究,掌握健康管理应用和慢病管理的未来发展趋势。明确慢病系统的设计目标是达到覆盖三类管理人群,形成三大业务闭环,使用三种业务手段;设计思路是以闭环业务体系为主线,形成三条业务路线,并能够根据区域特点进行业务扩展;整体架构为基础数据平台、数据采集平台、慢病检测平台和慢病管理平台;系统的关键技术包括感知技术、数据集成平台、评估模型。以达到国家对于慢病的有效监控和管理。
