摘要
目的分析产前超声检查诊断胎儿单脐动脉结合胎儿染色体核型分析对孕妇进行正确的产前咨询的价值。方法回顾性分析2011年1-12月在宁夏医科大学总医院行超声检查发现的18例单脐动脉,对其超声特征、胎儿染色体核型、产前咨询建议、妊娠结局进行随访分析。结果 18例胎儿单脐动脉病例中,其中16例单胎妊娠,2例双胎妊娠;9例单纯性单脐动脉,9例单脐动脉合并其他系统胎儿发育异常,其中染色体异常2例。随访结果中9例单纯性单脐动脉孕足月分娩,新生儿正常,8例引产,1例失访。结论单纯单脐动脉大多预后良好,产前咨询建议继续妊娠、定期随访;单脐动脉合并多发畸形者,若合并染色体异常,建议终止妊娠。临床医师应对胎儿单脐动脉进行正确的产前咨询,做好孕妇的孕期保健工作。
[ Objective ] To analyze the value of prenatal ultrasound diagnosis of fetal single umbilical artery combined with fetal karyotype analysis in prenatal counseling of pregnant women. [ Methods ] 18 fetuses with single umbilical artery were found by ultrasound screening from January to December, 201 lin our hospital. The sonographic features, fetal karyotype, prenatal advice and pregnancy outcome of the 18 cases were retrospectively analyzed. [ Results ] In 18 cases of single umbilical artery fetal, 16 cases were single pregnancy, 2 cases were twin pregnancy, 9 cases were simple single umbilical artery, 9 cases were single umbilical artery combined with fetal abnormalities of other systems, 2 fetuses＇ chromosome were abnormal. According to the results of follow-up, 9 cases of single umbilical artery were observed till full-term delivery, the newborns were normal, 8 cases were induced labor, 1 case was lost. [ Conclusion ] The majority of patients with simple single umbilical artery have a better outcome, the advice of continued gestation and regular follow-up recommendations can be provided to pregnant women. The fetus with single umbilical artery combined with multiple malformation and chromosomal abnormalities should be induced labor. Clinicians should supply proper prenatal consultation for the patients with single umbilical artery and do a good job of pregnant women＇s prenatal care.
