摘要 目的探讨胎儿纤维连接蛋白(fFN)、可溶性细胞间黏附分子-1(sICAM-1)及宫颈结构变化在临床早产预测中的价值.方法选取2013年3月~2014年2月因先兆早产在本院住院安胎的患者80例作为观察组,另选取同时期在本院门诊产检的孕妇80例作为对照组.观察两组的fFN检测情况,比较fFN阳性者中不同宫颈长度组间的早产率及早产孕妇与正常孕妇之间sICAM-1水平.结果观察组的fFN阳性率显著高于对照组,差异有统计学意义(χ^2=44.0580,P=0.0000);宫颈长度<30mm组的早产率明显高于宫颈长度>30mm组,差异有统计学意义(χ^2=38.1203,P=0.0000);早产孕妇的sICAM-1水平显著高于正常孕妇,差异有统计学意义(t=14.4786,P=0.0000).结论fFN、sICAM-1及宫颈结构变化在临床早产预测中具有一定的临床应用价值. Objective To investigate the predictive value of fetal fibronectin (fFN),soluble intercellular adhesion molecule-1 (sICAM-1) and cervical structure changes on preterm.Methods 80 cases of patients in our hospital because of threatened premature labor from March 2013 to February 2014 were selected in the observation group,and 80 cases of pregnant women at the same period in our hospital were selected in the control group.fFN testing situation between two groups,the rate of preterm birth between two groups of different cervical length in fFN positive women and the sICAM-1 level between preterm and normal pregnant women were observed.Results fFN positive rate of the observation group was significantly higher than that of the control group,and the difference was significant (χ^2=44.0580,P=0.0000); The rate of preterm birth in cervical length〈30 mm group was higher than that of cervical length〉30 mm group,and the difference was significant (χ^2=38.1203,P=0.0000); The sICAM-1 level of the preterm was higher than that of normal pregnant women,the difference was significant (t=14.4786,P=0.0000).Conclusion fFN,slCAM-1 and cervical structure changes has some clinical value in predicting preterm.
