摘要
目的了解血液科住院患者医院感染的发生率,分析医院感染的主要病原菌,为预防或减少医院感染的发生提供参考依据。方法对血液科2011年1月-2012年3月的562例住院患者临床资料进行收集并系统分析,采用自行设计问卷调查医院感染患者的基本情况、医院感染部位、病原菌的分布,采用SPSS17.0进行数据的统计分析与处理。结果血液科562例住院患者发生感染52例,千日感染率5.9‰;感染部位主要为呼吸道,占53.7%;检出病原菌中以革兰阴性菌为主占43.2%,其次为革兰阳性菌占37.9%,真菌占18.9%。结论恶性血液病患者易发生医院感染,在加强室内空气、设备消毒基础上,慎重选择预防性抗菌药物,减少侵入性操作,缩短住院时间,对于降低血液科病房感染有重要临床意义。
OBJECTIVE To investigate the infection rate in hematology wards and analyze risk factors for hospital infection so as to provide reference for prevention or decrease of hospital infections. METHODS The clinical data of 562 inpatients in the hematology department during Jan. 2011 to Mar. 2012 were collected and systematically ana- lyzed. The self-made questionnaire was used to investigate the general information of infected and uninfected patients, the main infected site, the distribution of pathogens and potential influencing factors. Data were pro- cessed and analyzed using the statistical software SPSS 17.0. RESULTS Fifty-two patients had infections among the 562 inpatients in the hematology department, the incidence of every 1000 days was 5.9%o. The main site of in- fection was respiratory tract （53. 7%）. Gram-negative bacteria were the main pathogen （43. 2%）, followed by gram-positive bacteria （37.8%） and fungus （18.9 %）. CONCLUSION Patients with hematologic malignancies are more likely to have hospital infection. On base of strengthening disinfection of the air and the equipment in the ward, it has important clinical significance to carefully choose prophylactic antibiotics, reduce invasive operation, and shorten hospitalization time for reducing the occurrence of infection in hematology wards.
