弹力压迫硅胶喷剂外喷及局部曲安奈德注射防治烧伤后瘢痕形成的探讨 被引量:4

摘要 目的探讨烧伤后瘢痕防治的有效方法。方法 98例2 880个部位采用局部弹力压迫加创面外用硅胶喷剂;对332例,352个部位面积较小者采用局部注射2%利多卡因+曲安奈德,注射后再用硅胶喷剂外喷加弹力压迫。对82例瘢痕挛缩致关节畸形者行瘢痕松解植皮或皮瓣松解术。结果 430例患者3 232个部位,显效1 357个部位,占42%;有效970个部位,占30%;总有效率为72%,无效905个部位,占28%。结论弹力压迫、硅胶喷剂创面外喷及局部注射曲安奈德是防治烧伤后瘢痕增生的有效方法。 Objective To explore the effective method to scar prevention after burn. Methods A total of 2 880 wound sites from 98 patients were treated by local elastic compression plus topical silicone spray; another 352 smaller wound sites from 332 patients were treated by local injection of 2% lidocaine plus triamcinolone acetonide, followed by topical silicone spray plus elastic compression. Skin graft or flap release was applied to the 82 cases of joint deformities caused by sear coutracture. Results Of the total 3 232 burned sites ( from 430 patients), 1 357 (42%) were markedly effective and 970 (30%) were effective, with a total effective rate of 72%. On the other hand, 905 sites (28%) were ineffective. Conclusion Local elastic compression plus topical silicone spray, together with local injection of triamcinolone acetonide, is an effective method to scar prevention after burn.
