摘要
目的:探讨血中性粒细胞与淋巴细胞比值(neutrophiltolymphocyteratio,NLR)在单纯冠状动脉扩张(coronaryarteryectasia,CAE)患者中的临床意义。方法:根据冠状动脉造影结果,人选53例单纯CAE患者[单纯CAE组,平均年龄(53±13)岁]和64例冠状动脉造影正常者[对照组,平均年龄(52±8)岁],记录两组血白细胞计数、中性粒细胞计数、淋巴细胞计数,计算NLR,并进行组间比较;分析NLR与其他炎性指标及CAE的扩张程度及分布类型的相关关系;多因素分析单纯CAE的独立预测因素。结果:与对照组比较,CAE组的NLR明显升高(3.055±1.461vs2.175±0.963,P〈0.001)。且随着NLR水平的增加,单纯CAE的诊断比例(P〈0.001)、白细胞计数(P=0.001)、高敏C反应蛋白(P=0.003)明显增加。相关分析显示NLR与白细胞计数呈正相关(r=0.523,P〈0.001),而与高敏C反应蛋白(P=0.292)、CAE的扩张程度及分型(P=0.577既关。多因素分析显示,NLR为单纯CAE的独立危险因素(比值比:2.277,95%可信区间:1.130~3.958,P=0.004o结论:NLR在CAE患者中升高,是CAE的独立危险因素。
Objective: To explore the clinical significance patients with isolated coronary artery ectasia (CAE). of neutrophil to lymphocyte ratio (NLR) of peripheral blood in Methods: According to coronary angiography (CAG) result, our study included 2 groups, CAE group, n=53 patients at the mean age of (53± 13) years and Control group, n=64 subjects with normal coronary artery at the mean age of (52+8) years. The counts of white blood cells (WBC), neutrophil and lymphocyte were recorded, NLR was calculated, and the relationship between NLR and CAE was investigated in both groups. The independent predictors for CAE were analyzed. Results: Compared with Control group, NLR was significantly higher in CAE group, (2.175±0.963) vs (3.055±1.461), P〈0.001. With the elevated NLR value, the diagnosis rate of isolated CAE, WBC count and high sensitivity C-reactive protein (hs-CRP) level were increased accordingly, P〈0.001, P=0.001 and P=0.003 respectively. The relevant analysis indicated that NLR was positively related to WBC count (r=0.523, P〈0.001), but not related to hs- CRP level and the severity/type of CAE, P=0.292 and P=0.577 respectively. Logistic regression analysis presented that NLR was the independent risk factor for CAE diagnosis (OR: 2.277, 95%CI: 1.130-3.958, P=0.004). Conclusion: The patients with isolated CAE has the increased inflammatory index NLR, it is the independent risk factor for isolated CAE diagnosis.
