摘要
目的 探讨阿托伐他汀对急性心肌梗死(AMI)后室性心律失常的抑制作用及机制.方法 90只雄性AMI大鼠均随机分为高、低剂量阿托伐他汀组及AMI组(n=30只),每组大鼠再均分为第3天、第1周2个亚组(n=15只).另取20只大鼠作为假手术组,均分为第3天、第1周2个亚组(n=10只).高、低剂量阿托伐他汀组每日给予阿托伐他汀20 mg/(kg·d)、10 mg/(kg·d)灌胃1周,假手术组和AMI对照组不予治疗.第3天、第1周末时以程序电刺激诱发各组大鼠室性心律失常后,假手术组取位于左心室游离壁心肌组织,心肌梗死各组取位于左心室梗死边缘带心肌组织检测 IL-6表达水平.结果 第3天、第1周末时AMI组梗死边缘带 IL-6表达水平及室性心律失常诱发率均高于假手术组(P〈0.01).第1周末时低剂量阿托伐他汀组梗死边缘带IL-6表达水平及室性心律失常诱发率显著低于AMI组(P〈0.05),而第3天时则差异无统计学意义(P〉0.05).第3天、第1周末高剂量阿托伐他汀组梗死边缘带IL-6表达水平及室性心律失常诱发率显著低于AMI组(P〈0.01).高剂量阿托伐他汀组梗死边缘带IL-6表达水平及室性心律失常诱发率在第3天、第1周末时均显著低于低剂量组(P〈0.01或P〈0.05).结论 阿托伐他汀可通过抑制IL-6过度表达而降低室性心律失常发生率,其作用呈剂量依赖性.
Objective To investigate the inhibitory effect of atorvastatin against ventricular arrhythmia after acute myocardial infarction (AMI). Methods Ninety male AMI rats induced by left anterior descending branch ligation were randomly divided into AMI group, high dose atorvastatin treatment group and low dose atorvastatin treatment group equally ( n = 30). Every group was further divided into 3 d and 1 week subunits equally ( n = 15 ). Twenty rats were taken as the sham operation group, and were further divided into 3 d and 1 week subunits equally (n = 10). The rats in high and low dose group were respectively treated with 20 mg/( kg ~ d) and 10 rag/( kg ~ d) of atorvastatin for 1 weeks by garage, whereas the rats in sham operation group were not given with any treatment. By the end of the 1 st week, programmed electrical stimulus was imposed on the rat hearts to induce ventricular arrhythmia. Then the left ventricle free wall tissue in the sham operation group and the border zone myocardium in the AMI rats were taken to determine the expression of myocardial IL - 6. Results Compared to the sham operation group, there were higher expression of myocardial IL -6 and the induced ventricular arrhythmia at 3 d and 1 week in the AMI group. Compared to the AMI group, low dose atorvastatin treatment reduced the expression of myocardial IL - 6 and the induced ventricular arrhythmia at 1 week ( P 〈 0.05 ), but there was no difference on the expression of myocardial IL - 6 and the induced ventricular arrhythmia at 3 d. Compared to the AMI group, high dose atorvastatin treatment reduced the expression of myocardial IL - 6 and the induced ventricular arrhythmia at 3 d and 1 week (P 〈 0.01 ). Compared to the low dose atorvastatin group, high dose atorvastatin treatment reduced the expression of myocardial IL - 6 and the induced ventricular arrhythmia at 3 d (P 〈 0.0t ) and 1 week (P 〈 0.05 ). Conclusion Atorvastatin may dose - dependently reduce the incidence of ventricular arrhythmia
