摘要
目的评估一种带有环状标测导管的新型冷冻球囊技术用于心房颤动导管消融的可行性和有效性。方法入选2009年1月至2011年3月德国柏林心脏中心就诊的52例症状性心房颤动患者(41例阵发性心房颤动,11例持续性心房颤动),采用这种带有环状标测导管的新型冷冻球囊技术进行心房颤动消融治疗。结果共184支肺静脉(184/208,88.5%)被成功隔离。总术程时间为(157.8±42.6)min,曝光时间为(36.6±14.2)min,总消融时间为(38.9±13.8)min。随访(12±8)个月后,31例(59.6%)患者成功维持窦律。仅有1例患者在消融右上肺静脉时出现膈神经麻痹并在3个月后完全恢复。无一例出现肺静脉狭窄。结论这种新型冷冻球囊导管消融技术安全有效,可简化PVI操作。
Objective To investigate the feasibility and efficacy of a simplified cryoballoon technique in which a microcircu- lar mapping guide wire is introduced into the central lumen of the cryoballoon catheter. Methods Fifty-two patients ( aged 60± 111 years) with symptomatic atrial fibrillation (AF) 41 cases for paroxysmal AF and 11 cases for persistent AF were enrolled. A simplified cryoballoon technique in which a microcircular mapping guide wire was introduced into the central lu- men of the cryohalloon catheter was used for mapping and ablation. Results A total of 184 PVs ( 184/208,88.5% ) were completely isolated. The total procedural time was ( 157.8 ± 42. 6 ) minutes ; the fluoroscopy time was ( 36. 6 ± 14. 2 ) mi- nutes. Total eryoablation time was (38.9 ± 13.8 ) minutes per patient. At follow-up ( mean [ 12 ± 8 ] months ), 31 (59.6%) patients were free from atrial fibrillation. There was one occurrence of phrenic nerve palsy during the cryoablation in the fight superior pulmonary vein ,which fully resolved after three months. Conclusion The use of this novel eryoballoon tech- nique appears to be safe and effective, and may simplify the ablation procedure for AF.
