摘要
目的:探讨血清基质金属蛋白酶(MMP-2)在急性冠脉综合征(ACS)发病中的作用及其与冠心病病变程度的关系。方法:选择100例冠心病患者,其中稳定型心绞痛(SAP)患者50例(SAP组)和ACS患者50例(ACS组),另选20例健康体检者作为健康对照组。检测各组患者的血脂,血清高敏C反应蛋白(hsCRP)、心肌肌钙蛋白I(eTnI),MMP-2水平,并进行比较分析。结果:(1)与健康对照组比较,ACS组和SAP组甘油三酯水平显著升高,高密度脂蛋白水平显著降低;cTnI[(0.011±0.024)μg/L、(O.019±0.026)μg/L比(1.113±4.220)μg/L]水平显著升高,且ACS组cTnI水平显著高于SAP组(P〈0.05~〈0.01);(2)与健康对照组和SAP组比较,ACS组hsCRP[(0.59±0.32)pg/ml、(1.40±1.90)pg/ml比(4.79±4.31)pg/ml]水平、MMP-2[(0.30±0.14)pg/L比(0.38±0.78)μg/L比(0.58±0.54)μg/L]水平显著升高(P均〈0.01);(3)Pearson线性相关分析显示,各组血清MMP-2与血清hsCRP具有明显正相关(r=0.374,P〈0.01)。结论:MMP-2水平升高可能与急性冠脉综合征的发病有关,MMP-2水平检测对急性冠脉综合征的早期诊断和治疗具有一定的应用价值。
Objective.. To explore role of serum matrix metalloproteinase - 2 (MMP - 2) in onset of acute coronary syndrome (ACS) and its relationship with diseased degree of coronary heart disease (CHD). Methods: A total of 100 CHD patients were selected, including 50 cases with stable angina pectoris (SAP, SAP group) and 50 ACS patients (ACS group). Another 20 healthy subjects were enrolled as healthy control group. Levels of blood lipids, serum high sensitive C reactive protein (hsCRP), cardiac troponin I (cTnI) and MMP- 2 were measured, compared and analyzed in all groups. Results: (1) Compared with healthy control group, there was significant increase in triglyceride level, significant decrease in high density lipoprotein level; and significant increase in cTnI [ (0. 011 ± 0. 024) μg/L vs. (0. 019 ± 0. 026)μg/L vs. (1. 113 ± 4. 220) μg/L] in SAP group and ACS group, furthermore the cTnI level of ACS group was more than that of SAP group P〈0.05-〈0.01; (2) Compared with healthy control group and SAP group, the hsCRP [ (0.59 ± 0.32) pg/ml, (1.40 ± 1.90) pg/ml vs. (4.79 ± 4.31) pg/ml], MMP- 2 [ (0.30 ± 0.14) μg/L, (0.38 ± 0.78)μg/L vs. (0.58 ± 0.54) μg/L] level significantly increased (P〈0. 01 all) in ACS group; (3) Pearson rectilinear correlation analysis indicated that serum MMP-2 was positively correlated with serum hsCRP (r = 0. 374, P(0.01) in all groups. Conclusion.. Increased MMP- 2 level may be related with occurrence of acute coronary syndrome, and its detection possesses certain application value for early diagnosis and treatment of acute coronary syndrome.
