摘要
目的探讨1.5T场强下磁共振扩散加权成像诊断前列腺癌的能力,以及ADC值与Gleason评分的关系。方法根据前列腺穿刺结果,回顾性分析32例患者的前列腺扩散加权图像中相对应的187个感兴趣测量区,其中癌肿灶102个,良性灶85个。测量信号强度及ADC值,并进行统计学分析,并利用受试者工作特征(ROC)曲线进行评价。通过直线回归分析探讨ADC值与Gleason评分之间的关系。结果癌肿组和良性组在b=0、100、800 s/mm2时的信号强度、ADC值的差异有统计学意义(P<0.05),根据ROC曲线,ADC值的诊断能力最强。ADC值随着癌肿Gleason分值的升高而降低。结论在1.5T场强下,扩散加权成像特别是ADC值对前列腺癌具有较强的诊断能力,且能够为判定肿瘤分化程度提供依据。
Objective To examine the ability of diffusion weighted imaging(DWI) at 1.5T MR imaging in diagnosing prostate cancer and correlation between apparent diffusion coefficient (ADC) value and Gleason score (GS). Methods Based on biopsy results, we retrospectively measured 187 regions of interest (ROI) from 32 patients, composed of 102 cancer lesions and 85 non-cancer lesions. Statistical analysis methods for the DWI data included receiver operating characteristic (ROC) curve and linear regression analysis. Results Significant differences were found between the two groups (P 〈 0. 05 ) in signal intensities (b = 0,100,800 s/mm2) and ADC values. According to ROC curves, the ADC value appears to have the highest diagnostic abili- ty. Linear regression analysis revealed an inverse relationship between ADC values and GSs: higher GS lesions have accordingly lowered ADC values. Conclusion ADC values obtained by DWI with 1.5T MR imaging is valuable in diagnosis of prostate canc- er and can provide indicative evidence for tumor differentiation analysis.
