摘要 目的:探讨磁共振全身弥散成像(WB-DWI)在前列腺癌(PCa)转移灶筛查中的应用。材料与方法:收集47例前列腺癌病例,均行全身弥散成像,结合病变部位常规MRI扫描;其中盆腔39例、全脊柱30例、胸部5例、中上腹部7例,取T1、T2加权像及压脂成像扫描。结果:本组病例筛查出骨盆转移33例,脊柱转移21例,双肺转移3例,肝脏转移2例,肾脏转移2例;骨盆、脊柱、双肺、肝脏均为多发转移,肾脏为单发转移。前列腺癌及转移灶在WB-DWI黑白反转图像上呈团块状、结节状、斑片状或斑点状低信号,T1WI呈稍低信号,T2WI/STIR呈稍高信号,肝脏转移灶增强后不规则环状强化。结论:全身弥散成像结合常规MRI扫描,对前列腺癌及转移灶能起到一站式检出诊断的作用。 Purpose: Investigate application of whole body diffusion weight imaging(WB-DWI) in detection prostate cancer(PCa) metastasis. Materials and Methods: 47 cases of Pea were collected, all cases underwent WB-DWI , com- bine with conventional MRI scan in lesions. The pelvic cavity ( 39 cases), spinal ( 30 cases), Thoracic (5 cases), abdo- men ( 7 cases) ; take T1 , T: weighted imaging and fat-suppressed imaging scan. Results: In the group of cases detected pel- vic metastasis (33 cases), spinal metastasis ( 21 cases), lung metastasis ( 3 cases), liver metastasis ( 2 cases), renal me- tastases ( 2 cases) ; pelvis, spine, lung and liver were mutiple metastases, kidney is solitary metastasis. PCa of the prostate and metastasis in the WB-DWI inversion image is mass, nodular, patchy or mottled low signal, T1WI showed slightly low sig- nal, slightly high signal of T2WL/STIR, hepatic metastasis after enhancement show irregular ring-like enhancement. Conclu- sion: Whole body diffusion weight imaging combine conventional MRI scan, can play one station type detection and diagnosis function in PCa of the prostate and metastasis.
