摘要
目的了解尘肺病诊断新标准实施前后郴州市尘肺病新病例诊断情况,为提高尘肺病诊断质量提供依据。方法收集郴州市疾控中心2008-01-01/2012-12-31尘肺病病例诊断资料,对全部初次诊断病例进行回顾性分析。结果2008-2012年诊断尘肺病新病例627例,全部为男性,平均年龄49.24岁,平均接尘工龄16.38年。其中壹期233例、贰期278例、叁期116例;煤工尘肺361例,矽肺111例,石墨尘肺5例,未分种类150例。尘肺病诊断新标准实施前后身份证明、用人单位职业史证明、工作场所检测结果提交、发现异常至确诊时间、诊断命名完整性等方面差异有统计学意义;明确接触粉尘名称、评定胸片质量、尘肺分期准确性等方面差异无统计学意义。结论尘肺病诊断存在的主要问题是未明确尘肺病人接触粉尘名称、未评定胸片质量。尘肺病诊断人员应加强对诊断标准的学习,严格执行标准规定,进一步提高诊断质量和水平。
Objective To investigate the diagnostic status of new pneumoconiosis patients in Chenzhou city before and after implementation of the new diagnostic criteria of pneumoconiosis,and provide scientific basis for improving diagnostic of pneumoconiosis. Methods Data of first diagnosed pneumoconiosis cases from January 1,2008 to December 31,2012were collected in Chenzhou Municipal Center for Disease Prevention and Control,and were analyzed by SPSS 17. 0.Results A total of 627 male cases were first diagnosed as pneumoconiosis during 2008 to 2012,including 233 stage one cases,278 stage two cases and 116 stage three cases. The average onset age was( 49. 24 ± 8. 74) years and the average dust exposure years was( 16. 38 ± 8. 14). The main type was coal worker pneumoconiosis( 361),followed by silicosis( 111) and graphite pneumoconiosis( 5),and 150 cases was undifferentiated type. Statistically significant difference were showed before and after the implementation of the criteria in terms of submitting of identification paper,history proves by work unit,detection of workplace,the time from found abnormal to confirmed and the integrity of diagnosis name; while no statistically significant difference were found in terms of ensuring the name of dust exposure,assessment the quality of films and the accuracy of pneumoconiosis stage. Conclusion The main problems in diagnostic of pneumoconiosis include unclear name of dust exposure and without assessment the quality of films. Physicians in pneumoconiosis diagnosis should advance the study of diagnostic criteria,and strictly enforce the standards and thereby improve the quality of pneumoconiosis diagnosis.
