降钙素原在慢性阻塞性肺疾病急性加重期患者中的价值 被引量:8

摘要 目的探讨血清降钙素原(PCT)检测在慢性阻塞性肺疾病急性加重期(AECOPD)患者中的临床价值。方法检测186例AECOPD患者PCT含量,观察治疗过程中PCT含量监测与患者病情的相关性,观察PCT含量在不同病原体感染患者中的差异,观察PCT含量对指导抗菌药物应用的意义。结果 186例AECOPD患者治疗前PCT含量(11.6±1.4)μg/L,治疗后呈逐渐下降趋势,与患者白细胞总数及体温下降程度呈正相关(r=0.898、0.762,P<0.05)。细菌感染患者PCT含量[(13.8±1.6)μg/L]显著高于病毒感染患者[(2.8±0.4)μg/L,t=69.013,P<0.01]及特殊病原体感染患者[(2.6±0.3)μg/L,t=73.155,P<0.01]。革兰阴性菌感染患者PCT含量[(14.6±2.3)μg/L]显著高于革兰阳性菌[(9.6±1.8)μg/L,t=22.993,P<0.01]。以PCT指导抗菌药物使用时间[(4.82±1.12)d]显著低于WBC及体温指导应用抗菌药物时间[(7.25±2.13)d,t=8.450 6,P<0.05],并且缩短了住院时间[(6.28±2.13)d和(8.69±2.11)d,t=6.922 9,P<0.05]。结论血清PCT含量可判断AECOPD患者的病情,并且有助于鉴别病原体感染类型,PCT指导AECOPD患者抗菌药物的应用可缩短使用时间。
