摘要
目的探讨控制成骨细胞分化的关键因素BMP-2及炎症信号受体TLR-4与骨折创伤后感染性炎症反应的关系。方法 18只雄性新西兰兔随机分为对照组(C组)、单纯开放骨折组(O组)和开放骨折+感染组(OI组),每组6只。O组利用手术方法制备胫骨开放骨折模型,OI组在O组的基础上用金黄色葡萄球菌感染造成开放骨折合并感染模型。术后第4天测定胫骨断端组织BMP-2和TLR-4基因表达情况(mRNA和蛋白水平)。结果与C组比较,O组BMP-2和TLR-4 mRNA和蛋白水平均显著升高,OI组BMP-2 mRNA降低,而TLR-4 mRNA和蛋白升高;与O组比较,OI组BMP-2 mRNA和蛋白下降,TLR-4mRNA和蛋白增加。结论开放骨折感染早期时,BMP-2表达下调而TLR-4表达上调,对骨折愈合将起不利作用。提高骨折部位微环境BPM-2表达并抑制TLR-4表达可能是治疗策略的新靶点。
Objective It is to observe the expression of BMP -2 (key factor to regulate osteoblast differentiation) and TLR - 4 ( inflammation signal receptor) in open fracture plus infection model of rabbit and to investigate the relationship of BMP- 2, TLR- 4 and fracture posttraumatic infective inflammatory reaction. Methods Eighteen male New Zealand rabbits were divided into control (C) group, open fracture (O) group and open fracture plus infection (OI) group which has 6 ani- mals in each group. Tibia open fracture model was made by surgery in O group, while in OI group, open fracture concurrent infection model was conducted by staphylococcus aureus infected based on O group. BMP -2 and TLR -4 gene expression ( including mRNA and protein) of tibia was determined after 3 days of experiment. Results Compared with C group, BMP - 2, TLR -4 mRNA and protein level all increased in O group (all P 〈0.01 ) , BMP -2 mRNA decreased while TLR -4 mR- NA and protein raised ( all P 〈 0.01 ) in OI group; compared to O group, BMP - 2 mRNA and protein reduced (P 〈 0.05 and P 〈 0.01 respectively) , but TLR -4 mRNA and protein level increased (both P 〈 0.01 ). Conclusion On early stage of open fracture concurrent infection, BMP- 2 gene expression down-regulated while those of TLR- 4 up- regulated, which is detri- mental to the healing of bone fracture. It may be novel target that improve BPM -2 expression and suppress TLR -4 expres- sion in the microenvironment of fracture site.
