小剂量吗啡关节内注射用于膝关节镜术后镇痛的临床观察 被引量:14

摘要 目的 :观察小剂量吗啡关节内注射用于膝关节镜手术后的镇痛效果。方法 :实验采用随机分组、双盲研究。腰麻下行膝关节镜手术的病人 40例 ,随机分为吗啡组(n =2 0 )和对照组 (n =2 0 ) ,吗啡组在术后关节内注射小剂量吗啡 (1毫克 ) ,对照组注射等体积的生理盐水 ,给药后 8小时和 2 4小时 ,在病人双足站立膝关节伸展状态下进行视觉模拟评分。结果 :吗啡组评分平均为 1 .7± 1 0和 1 .5± 1 1 ,而对照组平均为3.6± 1 5和 2 .4± 1 3,吗啡组评分显著低于对照组 ,两组有显著差异 (P <0 .0 5 )。结论 :小剂量吗啡关节内注射可以有效缓解膝关节镜手术的术后疼痛。 Aim: To evaluate the analgesic effects of low dose intra articular morphine after knee arthroscopy. Method: In the double blind study,40 patients were randomized to receive either low dose morphine 1mg (morphine group,n=20) or the same volume of normal saline solution (control group,n=20) intra articularly at the end of arthroscopic knee surgery. Analgesic efficacy was evaluated by visual analogue scale (VAS) 8 and 24 hours after the administration of morphine or placebo. This was done with the patients standing on both legs with extended knees. Results: The VAS were 1.7±10 and 1.5±11 in morphine group, respectively, which were significantly less than 3.6±15 and 2.4±13 in control group, respectively (P<0.05). Conclusion: That low dose intra articular morphine can effectively relieve the pain after arthroscopic knee surgery.
