摘要
目的 探讨护理干预对乳腺癌改良根治术后即刻乳房重建患者疾病不确定感的影响.方法 选择2011年3月至2012年10月在我科接受乳腺癌改良根治术的患者20例为研究对象,10例接受乳腺癌改良根治术后行即刻乳房重建手术患者为观察组,10例接受乳腺癌改良根治术患者为对照组.对照组患者入院后采用一般的治疗及护理,观察组患者在一般的治疗及护理的基础上,应用个案管理模式进行护理干预.两组患者均采用疾病的不确定感量表（MUIS）进行干预前、后的测量,并对测量结果进行统计分析.结果 护理干预前、入院当天,观察组疾病不确定感为（97.100±3.900）,对照组为（96.900±2.923）,差异无统计学意义（P＞0.05）;护理干预后、出院当天,观察组疾病不确定感为（67.800±6.088）,对照组为（73.000± 1.687）,差异有统计学意义（P＜0.05）.结论 护理干预可明显降低乳腺癌改良根治术即刻乳房重建手术患者的疾病不确定感,提高其生活质量.
Objiective To investigate the influence of nursing intervention on the disease uncertainty of patients undertaking immediate breast-reconstruction surgery after modified radical mastectomy. Methods Twenty patients were selected from Mar. 2011 to Oct. 2012. 10 eases receiving immediate breast- reconstruction surgery after modified radical masteetomy were enrolled into the observation group and the other 10 eases receiving modified radical mastectomy were grouped as the control group. Standard nursing was received by the eontrol group, while standard nursing plus case management were given to the obsmwation group, Mishel＇ s uncertainty in illness scale （MUIS} was used to assess the effect of the intervention on all patients. Results Befin＇e the nursing intervention, the disease uncertainty rate was （97.100 ±3.900） in the observation gronp and was （96.900 ± 2.923） in the control group on the admission day, with no statistical difference （P〉0.05）. After the nursing intervention, the disease uncertainty rate was （67.800 ± 6.088） in the observation group and was （73.000 ± 1.687） in the control group on the discharge day, with a stalislical difference （P〈0.05）. Conclusions Nursing intervention with case management can significantly reduce the MUIS scores and improve the quality of life of the patients nnertaking immediale breast-reconstruction surgery after modified radical mastectomy.
