摘要
目的:比较社会心理因素对广西壮、汉族哮喘患者发病的影响。方法:收集壮族哮喘患者188例,汉族哮喘患者175例;另收集壮族健康人群203例,汉族健康人群180例。调查所有对象的家庭社会经济状况,心理测评则采用激惹、抑郁和焦虑自评量表(IDA)、特质应对方式问卷(TCSQ)及社会支持评定量表(SSRS)。结果:壮族哮喘患者抑郁、消极应对(NC)的评分高于汉族哮喘患者(P<0.05),社会支持度各维度在壮汉族哮喘患者中无差异(P>0.05)。多因素Logistic回归分析显示壮族人群有文化程度、哮喘家族史、抑郁、内向性激惹、NC及主观支持6个变量入选模型,而汉族人群有文化程度、吸烟、内向性激惹、主观支持4个变量进入模型,可见文化程度、内向性激惹、主观支持为壮、汉族人群共有的哮喘影响因素。结论:社会心理因素对广西壮、汉族哮喘患者发病影响存在一定的差异,可为探索哮喘的发病机制以及有针对性的哮喘防治提供科学的依据。
Objective To compare the influence of social-psychological factors on Guangxi Zhuang and Han patients with asthma. Methods We enrolled 188 Zhuang patients with asthma as the case group and 203 healthy people as the control group, as well as 175 Han patients with asthma as the case group and 180 healthy people as the control group. The family and socio-economic status of the enrolled patients were investigated, and the psychological tests were assessed by irritability, depression and anxiety scale （IDA）, trait coping style questionnaire （TCSQ） and social support rating scale （SSRS）. Results The scores of depression, anxiety negative coping （NC）, external irritation and internal irritation in Zhuang patients were significantly higher than those of Han patients （P 〈 0.05, respectively）, however, no significant difference was shown in the dimension of social support between Guangxi Zhuang and Han patients with asthma （P 〉 0.05）. Result of multivariate logistic regression analysis showed that the cultural level, family history of asthma, depression, irritability, NC and subjective support were independent risk factors for the Zhuang patients with asthma, whereas the cultural level, smoking, irritability, and subjective support were independent risk factors for the Han patients with asthma. Conclusion The Social- psychological factors have different effects on the Guangxi Zhuang and Han patients with asthma. This study provides a scientific basis not only for exploring the pathogenesis of asthma, but also for preventing and treating asthma.
