摘要
目的观察维生素C(VitC)透析液对维持性血液透析(MHD)患者微炎症状态及促红细胞生成素(EPO)疗效的影响。方法将行MHD的45例慢性肾功能衰竭患者按随机数字表法分为常规血液透析组(A组)、VitC透析液2g·L-1组(B组)及VitC透析液4g·L-1组(C组),每组15例。MHD治疗前及治疗12周后检测各组超敏C-反应蛋白(hs-CRP)、IL-6水平及Hb值。结果 MHD治疗前3组hs-CRP、IL-6水平差异均无统计学意义(P>0.05)。经过12周MHD治疗后,B、C组hs-CRP、IL-6水平较治疗前均有下降,与A组比较差异均有统计学意义(P<0.05)。C组比B组治疗后hs-CRP、IL-6水平下降更明显(P<0.05),A组治疗前后hs-CRP、IL-水平差异无统计学意义(P>0.05)。Hb的升高与hs-CRP、IL-6水平均下降相关(r=0.26,P<0.05)。结论 MHD患者存在着微炎症状态,VitC透析液可有效改善MHD患者的微炎症状态并能提高EPO纠正贫血的效果。
Objective To investigate the effects of dialyzer with vitamin C on microinflamma- tion status and efficacy of erythropoietin in maintenance hemodialysis （MHD） patients. Methods Forty-five patients who underwent MHD for chronic renal failure （CRF） were randomly divided into three groups：conventional hemodialysis（group A） , dialyzer with 2 g vitamin C（group B） and dialyzer with 4 g vitamin C（group C）,with 15 patients in each group. Serum levels of high-sensi- tivity C-reactive protein（hs-CRP）, interleukin-6 （IL-6） and Hb were measured before and after treatment for 12 weeks. Results There were no significant differences in hs-CRP and IL-6 levels among the three groups before MHD treatment（P〉0.05）. After treatment for 12 weeks, dialyzer with vitamin C significantly decreased the levels of hs-CRP and IL-6, compared with conventional hemodialysis （P〈0.05）. Moreover, hs-CRP and IL-6 levels significantly decreased in group C, compared with group B（P〈0.05）. No obvious changes in the levels of hs-CRP and IL-6 were found after MHD treatment in group A（P）0.05）. The increase in haemoglobin was associated with the decrease in serum levels of hs-CRP and IL-6（r= 0.26, P%0.05）. Conclusion Microin- flammation state exists in MHD patients. The dialyzer with vitamin C can effectively ameliorate the microinflammation state and improve the effect of erythropoietin on anemia in MHD patients.
