摘要
目的 观察胆肠吻合愈合过程的超微结构变化 ,阐明良性胆管狭窄形成机制。方法 通过制作犬胆总管十二指肠吻合模型 ,分别于术后 1周、3周、3月、6月取材行透射电镜 (TEM )、扫描电镜 (SEM)观察 ,并行计算机图像分析测定细胞外胶原容积密度 (ECVD)变化。结果 胆道粘膜上皮修复较差 ,肌成纤维细胞功能活跃 ,持续存在于整个愈合过程 ,细胞外基质过度沉积。结果导致瘢痕性挛缩 ,吻合口狭窄。
Objective To observe ultrastructure changes of healing process after bilioenteric anastomosis and clarify the mechanism of benign biliary stricture formation.Methods A model of choledochoduodenostomy in the dog was constructed,then anastomosis tissue on 1,3 weeks and 3,6 months after operation were observed with TEM and SEM respectively.The changes of extracellular collagen volume density (ECVD) were determined by computer image analysis at the same time. Results The epithelium of mucosa membrane of bile duct restored badly,myofibroblasts functioned actively and lasted for the whole process,extracellular matrix overdeposited.The results leaded to scar contracture and anastomotic stenosis.Conclusion Myofibroblast is the main cause to result in scar contracture of bilioenteric anastomosis.
