摘要
目的:探讨血清TRAb测定水平在Graves病诊治中的价值。方法:本文采用放射免疫分析法测定TRAb同时用化学发光法测定sTSH、FT3、FT4含量,对71例符合诊断标准的Graves病患者治疗前,治疗后6个月、24个月血清TRAb水平及sTSH、FT3、FT4含量的变化与40名健康对照组进行比较。结果:治疗前血清TRAb水平、阳性率明显高于对照组,差异具有显著性(P﹤0.05);治疗后血清TRAb水平、阳性率明显低于治疗前,差异具有显著性(P<0.05),而仍明显高于对照组水平,差异具有显著性(P﹤0.05)。治疗后6个月血清TRAb水平、阳性率明显高于治疗后24个月水平,差异有显著性(P<0.05);对照组血清TRAb水平、阳性率与治疗后24个月水平差异无显著性(P>0.05)。对照组与治疗后sTSH、FT3、FT4含量的变化进行比较差异无显著性(P>0.05)。结论:Graves病患者甲状腺功能恢复快,血清TRAb转阴缓慢,血清TRAb水平、阳性率在判断疗效上具有重要意义;治愈后TRAb在一定时间内仍可阳性,TRAb转为阴性作为治愈疗效评价标准,在临床中的价值有待进一步探讨及重新评价。
Objective： To approach clinical application value of serum thyrotrophin receptor antibody（ TRAb） in diagnosis of Graves disease. Methods： In this text,radio immunoassay（ RIA） was taken to detect serum TRAb and chemiluminescence assay was taken to detect the content of serum sTSH,FT3,FT4. The 71 Graves disease patients were divided to pretherapy patients and post-treatment 6months patients,pretherapy patients and post-treatment 24 months patients. The variation of contents of serum TRAb and sTSH,FT3,FT4 between two groups and 40 healthy control group was compared. Results： The serum TRAb level and masculine rate of pretherapy patients were obviously higher than the control group. The difference had significance（ P 0. 05）; The serum TRAb level and masculine rate of post-treatment patients were obviously lower than pretherapy patients. The difference had significance（ P 0. 05）. But the serum TRAb level and masculine rate of post-treatment patients were also higher than the control group. The difference had significance（ P 0. 05）. The serum TRAb level and masculine rate of post-treatment 6 months patients were obviously higher than post-treatment24 months patients,the difference had significance（ P 0. 05）. The differences of serum TRAb level and masculine rate between the control group and post-treatment 24 months patients had no significance（ P 0. 05）. Conclusion： The Graves disease patients have fast recovery of thyroid function and the speed of serum TRAb level become normal slowly. The serum TRAb level and masculine rate have important sense in therapeutic effect judgement. The serum TRAb level will be higher than normal some time when the disease disappear. The clinical value which the serum TRAb level becoming normal is considerd the evaluation criterion of curing therapeutic effect need more approach.
