摘要
目的研究小剂量米非司酮联合女金片治疗围绝经期功能失调性子宫出血(功血)的临床效果及对内分泌功能的影响。方法将90例围绝经期功血患者随机分为观察组和对照组,分别给予小剂量(6.25 mg)米非司酮联合女金片治疗和大剂量(12.5 mg)米非司酮治疗,每组各45例,比较两组患者治疗前及治疗后6个月时子宫内膜厚度、月经情况、血清内性激素水平及肝肾功能。结果观察组临床疗效优于对照组(Z=-4.388,P<0.01);治疗后6个月观察组子宫内膜厚度为(2.77±0.26)mm,对照组为(3.59±0.35)mm,观察组子宫内膜厚度显著小于对照组(t=12.68,P<0.01);观察组与对照组卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、孕酮(P)水平较治疗前均有所下降;但观察组较对照组降低显著(t=19.39、6.92、32.31、5.36,均P<0.01);观察组患者在治疗停药后12个月有33例(73.33%)患者绝经,对照组有24例(53.33%)患者绝经,观察组的绝经率较对照组高(χ2=3.876,P=0.049)。结论小剂量米非司酮联合女金片能有效治疗围绝经期功血,副作用小,值得临床推广。
Objective To observe the effect of low-dose Mifepristone in combination with Nüjin Tablets on perimenopausal dysfunctional uterine bleeding and endocrine function.Methods Totally 90 patients with perimenopausal dysfunctional uterine bleeding were randomly divided into the observation group and control group,which were assigned to the treatment of lowdose （6.5 mg） Mifepristone and high dose （12.5 mg） Mifepristone combined with Nüjin Tablets,with 45 cases in each group respectively.The endometrial thickness,menstrual conditions,serum sex hormone levels,liver and kidney function before,and 6 months after treatment were compared between two groups.Results Clinical curative effect of observation group was significantly better than that of control group （Z =4.388,P 〈 0.01）; the endometrial thickness of observation group was （2.77±0.26） mm,which of the control group was （3.59±0.35） mm at 6 months after treatment,the observation group were less than the control group （t =12.68,P 〈 0.01）; the levels of FSH LH,E2,P were decreased after treatment in both groups,but all in the observation group were dropped significantly than those of control group （t =19.39,6.92,32.31,5.36,all P 〈 0.01）; 12 months after treatment,33 patients （73.33%） in observation group went into menopause,while the control group had 24 cases （53.33%）with menopause,the menopause rate of observation group was significantly higher than the control group （x2=3.876,P =0.049）.Conclusion Low-dose Mifepristone combined with Nüjin Tablets has definite effect,small side effects on the perimenopausal dysfunctional uterine bleeding,which is worthy of clinical promotion.
