摘要
目的探讨经阴道三维超声对不全纵隔子宫和弓形子宫的诊断价值。方法对宫腔镜和腹腔镜联合检查后确诊的不全纵隔子宫和弓形子宫各30例患者的经阴道二维和三维超声图像进行分析比较。结果宫腔镜和腹腔镜联合检查确诊的各30例不全纵隔子宫和弓形子宫中,术前经阴道二维超声准确诊断24例不全纵隔子宫和22例弓形子宫,诊断符合率分别约80.0%和73.3%;经阴道三维超声对不全纵隔子宫和弓形子宫的诊断符合率均为100%,二者比较差异有统计学意义(P<0.05)。结论经阴道三维超声能获得二维不能获得的冠状切面,可直观、准确地显示子宫外形轮廓及宫腔内膜形态,为诊断提供量化标准,明显提高了对不全纵隔子宫和弓形子宫的诊断符合率,可作为两者首选的检查方法。
Objective : To investigate the value of the diagnosis for incomplete mediastinal uterine and arcuate uterus with transvaginal three - dimensional ultrasound. Methods : Thirty cases of incomplete mediastinal uterine and 30 cases of arcuate uterus diagnosed by combined laparoscopic hysteroscopy were examined by two - dimensional and three - dimensional transvaginal ultrasound. The images were analyzed and compared. Resuhs : For thirty cases of incomplete mediastinal uterine and 30 cases of arcuate uterus diagnosed by combined laparoscopic hysteroscopy, preoperative two - dimensional accurately diagnosed 24 cases of incomplete mediastinal uterine and 22 cases of arcuate uterus, the diagonstic accordance rates of incomplete mediastinal uterine and arcuate uterus were 80.0% and 73.3% by transvaginal two -dimensional ultrasound while they were 100% by transvaginal three -dimensional ultrasound, which was statistically significant (P 〈 0. 05). Conclusion : Dimensional transvaginal ultrasound can get access to the coronal what two - dimensional can not, it can directly and accurately display the uterus contour and uterine endometrial shape, providing quantitative criteria for the diagnosis, it significantly improves diagonstic accordance rate on incomplete mediastinal uterine and arcuate uterus, therefore it can be used as the preferred method of diagnosis for incomplete mediastinal uterine and arcuate uterus.
