摘要
目的随着超声刀在临床外科中的应用及微创外科的发展,具有损伤小、痛苦少、恢复快等特点的微创手术一直是肛肠外科医师追求的目标。文章运用微创无血理念探讨FOCUS超声刀在痔病手术中的应用价值。方法将43例明确诊断为混合痔的患者随机分为试验组22例和对照组21例,排除手术禁忌后分别予采用FOCUS超声刀进行痔切除手术和采用电刀进行Milligan-Morgan手术,并观察2组患者的手术时间、术中出血量、创面疼痛程度、愈合时间、并发症发生情况等指标。结果 2组患者的手术时间、术中出血量、愈合时间及创面疼痛程度及术后并发肛门水肿、继发性出血等差异均有统计学意义(P<0.05);而术后并发尿潴留、肛门坠胀的差异无统计学意义(P>0.05)。结论 FOCUS超声刀在痔病手术中可缩短手术时间,有效减少出血量,明显缩短创面愈合时间,具有重要临床价值。
Objective Minimal Invasive operation is characterized by minimal invasion, less pain and faster recovery, which is increasingly adopted by coloproctological surgeons. This study aimed to investigate the value of Focus ultrasound scalpel in minimally in- vasive hemorrhoids operation. Methods A randomized controlled clinical trial was carried out, 43 patients with hemorrhoids were ran- domly divided into treatment group （n =22, Minimal invasive operation with Focus ultrasound scalpel） and control group （n = 21, M-M operation with electric scalpel）. The operation time, intraoperative bleeding, the severity of pain, healing time, complications and so on were observed and compared. Results The treatment group showed significantly shorter operation time, less intraoperative bleeding, short healing time and so on than the control group （ P 〈 0.05 ）. But there were no statistically significant differences in terms of urinary re- tention and anus bleated between the two groups （P 〉 0.05）. Conclusion The use of FOCUS ultrasonically scalpel in minimally inva- sire hemorrhoids operation can shorten operation time, reduce blood loss ; promote wound healing, and is worthy of promotion.
