摘要
目的对一例A2B亚型患者进行血型血清学和分子生物学检测,观察采血400 ml前后主要血液指标的变化。方法用抗-A、抗-A1、抗-B、抗-H单抗血清检测患者红细胞ABH抗原;用A、B、O型标准红细胞与患者血清反应检测ABO血型抗体及不规则抗体;利用聚合酶链反应扩增患者ABO基因6、7外显子编码序列并进行测序分析;采用血液分析仪检测血液常规指标。结果患者红细胞上有A、B抗原,血清中存在抗-A1抗体,符合A2B亚型的血型血清学特征;患者ABO基因6、7外显子直接测序分析显示基因型为A208/B101,单倍体测定为A208、B101两个等位基因。采血前后及回输前后血常规主要指标无明显差别。结论发现1例A208/B101个体,表现出A2B血清学特性,血清中存在抗-A1抗体;符合要求的自体采血前后血液主要指标不会产生明显的变化。
Objective To observe the changes of main blood index in a patient with A2 B subgroup before and after blood sam- pling based on blood group serologic test and molecular biology test. Methods The blood group ABH antigen of erythrocyte from the patient was detected by anti - A, anti - A1 , anti - B and anti - H antiserum. While the erythrocytes of A, B, O blood group reacted with serum of the patient to detect the ABO blood group antibody and irregular antibody. The coding region of exon 6, 7 of ABO gene was amplified by PCR for sequencing analysis, then blood analyzer was used to detect the conventional blood index. Results Both A and B antigen were found in erythrocyte of the patient, and anti - A1 in serum of patient was in line with the blood group serologic characteristics of A2 B subgroup. It can be assigned as A208/B101 genotype by directsequence a- nalysis for exon 6, 7 of ABO. A208 and B101 alleles were confirmed by ABO halpotype analysis. The main blood index had no obvious difference before and after blood sampling and transfusion. Conclusion An individual with A208/B101 was found, showing the serological characteristic of A2B subgroup with anti -A1 antibody in the serum. The main blood index had no obvious changes before and after the requested autologous blood collection.
