右心室心尖部和右心室流出道间隔部起搏对老年患者心功能的影响 被引量:8

摘要 目的探讨右心室流出道间隔部(RVOTS)起搏和右心室心尖部(RVA)起搏对老年患者心功能的影响,评价RVOTS起搏对老年患者的安全性。方法选择双腔起搏器置入的三度房室传导阻滞和快慢综合征老年患者57例,按心室电极固定部位分为RVA起搏组30例和RVOTS起搏组27例,记录术后心电图QRS波的宽度,比较2组术后1、3、9、15、21个月末LVEF、左心室舒张末内径(LVEDD)、血浆N末端B型钠尿肽前体(NT-proBNP)的差异。结果 2组术前,术后1、3、9个月LVEDD、LVEF比较,差异无统计学意义(P>0.05);RVA起搏组术后15、21个月,LVEDD和NT-proBNP较RVOTS起搏组明显升高,LVEF明显降低(P<0.01)。结论对于三度房室传导阻滞及快慢综合征老年患者行RVOTS起搏是安全可行的,对患者心功能影响小,是较为理想的起搏部位。
