摘要
目的探索全面提高医院药学服务质量的工作方法。方法查阅我院老年病房2010年1月至2012年12月584例住院患者的病历资料、药师工作记录、全部住院患者处方点评,统计试行"五师服务临床"模式前后科室合理用药指标、药师参加会诊、病例讨论、全程药学监护的例次,总结老年病房临床药师的工作模式和方法。结果我院老年病房临床药师工作与服务模式已初步建立,临床药师参与会诊、病例讨论、重点患者药学监护数量呈上升趋势。总药费、单日单床药费(元)、抗菌药物药费、抗菌药物使用强度、不合格处方率逐年下降。结论在我院倡导的"五师服务临床"模式下,临床药师对全面提高合理用药水平、避免医疗资源浪费、改善医疗质量有重要的促进作用,但其工作模式和绩效考评体系有待进一步完善。
Objective To explore an improved method to increase the quality of hospital service. Methods The survey was conducted among 584 hospitalized patients from January 2010 to December 2012. The quantity and quality of the performances of clinical pharmacist was analyzed such as rational drug use index, pharmacists work records,pharmacists consultations and case discussions. The working mode and method of clinical pharmacist in geriatric ward was summarized. Results Geriatric ward clinical pharmacists work and service model had been set. The quantity of pharmacists consultations, case discussions, pharmaceutical care patients increased. The pharmaceutical expenditure, antibiotics use density, unqualified prescriptions decreased year by year. Conclusion Clinical pharmacists have important roles in promoting rational drug use level, avoiding the waste of medical resources, increasing the quality of medical treatment under the mode of＂ five medical staffs＇clinical service＂ ,but their working modes and performance evaluation system should be further improved.
