摘要
目的探讨适合高危妊娠流产的有效流产方法。方法将2011年1月至2011年12月广州市妇女儿童医疗中心门诊手术室接诊的218例具有高危因素的早期妊娠并要求终止妊娠的患者分成药物流产组和B超引导下人工流产组,比较两组的流产结局、阴道出血量、流产后出血持续时间、血清人绒毛膜促性腺激素(β-HCG)恢复正常时间及术后感染率等临床指标,并进行统计学分析。结果药物流产组完全流产率明显低于B超引导下人工流产组,且差异有统计学意义(χ2=18.23,P<0.01)。药物流产组阴道出血量明显高于B超引导下人工流产组(t=12.98,P<0.01),术后出血持续时间、β-HCG恢复正常时间明显长于B超引导下人工流产组,差异有统计学意义(t=9.44,P<0.01)。药物流产组术后感染率与B超引导下人工流产组相比无明显差异。结论具有高危因素的早期妊娠行B超引导下人工流产术,比药物流产成功率高,出血量少,出血时间短,术后恢复快。
Objective To compare the two different abortion methods with early high-rlsk pregnancy, and discuss the effective way of the early high-risk pregnancy. Methods 218 cases of early high-risk pregnancy in Guangzhou women and children's medical center in 2011 were divided into medical abortion group and ultrasonic-guiding artificial abortion group at random, and all the cases were compared and analyzed, the outcome, vaginal bleeding volume, bleeding duration after abortion, the time of recovery of β-HCG and postoperative infectious rate by the statistic software SPSS 14. 0 respectively. Results The complete abortion rate in medical abortion group was lower than that in the ultrasonic-guiding artificial abortion group, and the differences was statically significant ( P 〈 0. 05 ) . The mean amount of vaginal hemorrhage in medical abortion group was much more than the ultrasonic-guiding artificial abortion group's, while the bleeding duration after abortion and the time of recovery of β-HCG were much longer. There were statistical difference between the two groups (P 〈 0. 05 ) . The incidence of postoperative infectious rates in medical abortion group was not significantly higher than that in the ultrasonic-guiding artificial abortion group (P 〉 0. 05) . Conclusions Ultrasonic-guiding artificial abortion is better than medical abortion in early high-risk pregnancy because its high success rate, tess vaginal hemorrhage, short bleeding duration and recovery time.
