摘要 目的探讨儿童神经纤维瘤病Ⅰ型患者脑部病变MRI表现特点。方法回顾性分析8例临床确诊的儿童NFⅠ患者脑部MRI资料。8例均行常规MRI平扫,其中3例行增强扫描,1例行双侧基底节病变波谱采集,分析病变好发部位、数目、信号特点、强化形式及波谱。结果 8例病变均为多发,主要累及小脑(7例),累及基底节区8例,脑干受累7例,4例累及丘脑,1例视交叉增粗,形态多为斑片状,2例合并圆点状病灶,呈等或稍长T1信号、稍长T2信号;3例增强扫描病灶无明显强化。1例双侧基底节区病灶波谱显示胆碱/肌酸(Cho/Cr)分别为0.86与1.24。结论儿童神经纤维瘤病Ⅰ型患者颅脑病变具有特征性MRI表现,常多发,同时累及小脑、脑干、基底节区,部分累及丘脑,T2WI上表现为高信号,无强化。MRI可全面显示颅脑多发病变,为临床诊断及随访提供重要的依据。 Objective To illustrate MR characteristic of brain lesions in children with neurofibromatosis type Ⅰ ( NFⅠ ). Methods MR images of 8 pediatric NF Ⅰ were reviewed. Contrast material was administered in 3 cases. 1 case performed Magnetic Resonance Spectroscopy. The MR images were analyzed with specific reference to the following: lesion numbers, location, morphology, signal characteristics , and presence or absence of contrast enhancement. Choline/Crea- tine (Cho/Cr)was calculated. Results Multiple hyperintense lesions on T2WI were found in the 8 patients. 7of lesions located in cerebellums,8 in the globus pallidus,7 in brainstems, 4 in halamus , 1 in chiasma. The lesions showed no con- trast enhancement. Cho/Cr of the bilateral globus pallidus was 0.86 and 1.24 in 1 case. Conclusion Brain MRI in chil- dren with NF I is characterized by muhiple lesions of prolonged T2 relaxation involving the brainstem, cerebellum, and globus pallidus, with no contrast enhancement. MRI can play important role in the diagnosis and follow up.
