摘要
目的探讨高频彩色多普勒超声监测股骨干骨折后骨痂血运的应用价值。方法采用高频彩色多普勒超声对25例股骨干骨折患者内固定术后1、2、3、4、8周骨折处骨痂形成的声像特征及其血运情况进行观察,并与X线结果对照。结果术后第1周,所有骨折部位周围出现血肿,其中7例血肿内见少许血流信号;术后第2周,骨折部位周围仍有少许血肿,骨折处未见骨痂回声,11例血肿内部观察到血流信号;术后第3周,所有患者骨折周围血肿基本消失,23例骨折部位见少许骨痂生长;术后第4周,23例骨折处骨痂饱满,X线显示骨痂开始增厚,2例骨折端未见骨痂连接;术后第8周,23例骨痂连接良好,2例骨折愈合不理想。结论高频彩色多普勒超声可清晰显示股骨干骨折术后骨痂形成及骨痂血运状况,可作为一种简便无创的监测手段。
Objective To discuss the application value of high-frequency color Doppler ultrasonography on the monitoring of fracture callus after fracture of femoral shaft. Methods By using high-frequency color Doppler ultrasound, the acoustic image characteristic and the blood supply of the callus formation in 25 patients with fracture of femoral after 1, 2, 3, 4, 8 weeks of internal fixation operation were observed. Results After 1 week, hematomas appeared around the fracture areas. 7 cases appeared with a little blood flow signals within the hematomas. After 2 weeks, a few hematomas still appeared around the fracture areas. There was no callus echo around fracture area. 11 cases appeared internal blood flow signals within the hematomas. After 3 weeks, hematomas of all patients nearly disappeared. 23 cases of fracture sites observed only a few growth of calluses. After 4 weeks, the filled calluses of 23 cases in fracture areas were observed. The X-ray result showed that the calluses started thickening. There was no callus observed in the end connection of 2 fracture cases. After 8 weeks, 23 cases of calluses connected well, with only 2 cases of unsatisfactory fracture healing result. Conclusions High-frequency color Doppler ultrasonography clearly shows the formation of femoral fracture callus and the blood supply condition of callus. It can be used as a simple and noninvasive way of monitoring.
