某院儿童住院患者万古霉素血药浓度监测及临床应用分析 被引量:20

摘要 目的分析应用万古霉素的儿童住院患者血药监测浓度,为临床合理用药提供参考依据。方法采用高效液相色谱法对患者应用万古霉素进行血药浓度监测,回顾性分析某院2011年1月-2012年7月万古霉素血药浓度监测结果及其相关信息。结果130例儿童感染患者的监测结果表明,平均谷浓度为(9.12~2.40)mg/L,谷浓度在〈5mg/L的例数占总体的51.54%,5~10mg/L32.31%,〉10mg/L的占16.15%;应用万古霉素治疗前后肝肾功能比较差异有统计学意义(P〈0.05);检出的致病菌81.16%为革兰阳性球菌,MRSA仅一例。结论该院儿童住院患者万古霉素临床应用存在部分不合理,临床应根据血药浓度与细菌培养、药敏结果积极调整用药,同时密切监测。肾功能变化。 Analyzing the drug monitoring concentration of blood in the pediatric hospital patients to provide a clinical evidence for rational administration. Methods Using HPLC to monitor blood concentration of vancomycin. A retrospective analysis of the result of cancomycin concentration was conducted in a central hospital from January 2011 to July 2012. Results In 130 cases of the pediatrical patients, the average though concentration of vancomycin was (9.12~=2.40)mg/L. The percent of cases blow 5mg/L was 51.54%, that of 32.31% was between 5 and 10mg/L and 16.15% was more than 10mg/L. There was significant differences in renal function before and after administration(P〈0.05). Pathogenic bacterias of 81.16% which was detected out were Gram-positive cocci. Only one MRSA was detected. Conclusion The usage of vancomycin in the pediatric hospitalized patients is certain irrational on the whole, vancomycin shoud be used as to the concentration and results of bacterial culture and sensitivity test. And monitoring kidney function change frequently.
