摘要
目的探讨膀胱癌患者手术前后血清中Dickkopf-1(DKK-1)的表达情况,及其与膀胱癌病理特征的联系。方法采用酶联免疫吸附试验(ELISA)测定48例膀胱癌患者(膀胱癌组)和24例输尿管结石或尿道肉阜患者(对照组)血清中DKK-1水平,采用Western免疫印迹法检测膀胱癌肿瘤组织、癌旁组织和正常组织中DKK-1、β-链蛋白的表达情况,同时采用免疫组织化学法检测DKK-1蛋白的表达阳性率。结果膀胱癌组术前的血清DKK-1水平为(8.18±2.83)ng/mL,显著高于术后的(6.06±2.27)ng/mL(P=0.021);且显著高于对照组的(4.86±2.56)ng/mL(P=0.007)。膀胱癌组低级别上皮癌与高级别上皮癌患者间,浅表性膀胱癌与肌层浸润性膀胱癌患者间手术前后的血清DKK-1水平的差异均无统计学意义(P值均>0.05)。膀胱癌组织和癌旁组织的DKK-1相对表达量分别为2.033±0.372、1.816±0.352,均显著高于正常组织的0.976±0.202(P值分别=0.018、0.047)。在膀胱癌组织中的DKK-1蛋白表达量明显高于对照组,且在血管周围组织有升高现象,但在间质中无明显升高。膀胱癌组织中DKK-1阳性率为48%,显著高于对照组的31%(P=0.020)。结论 DKK-1的表达与膀胱癌的发生关系密切,有可能成为一种新的肿瘤标志物,用于对膀胱癌的检测与诊断。
Objective To evaluate the expression of Dickkopf-1(DKK-1)in bladder cancer patients before and after surgery,and to further study its relationship with clinical pathological features.Methods Enzyme linked immunosorbent assay(ELISA)was used to detect serum DKK-1expression of 48cases in bladder cancer group and 24cases in control group.Western blot was used to detect protein expression of DKK-1andβ-catenin in bladder cancer tissues,adjacent tumor tissues and normal tissues.The differences of DKK-1expression between two groups were assayed by immunohiatochemistry,and the results were observed under Leica microscope.Results The preoperative DKK-1level in the bladder cancer group([8.18±2.83]ng/mL)was significantly higher than postoperative level([6.06±2.27]ng/mL,P=0.021)and that in the control group([4.86± 2.56]ng/mL,P =0.007).There were no statistical differences in serum DKK-1levels between primary epithelioma and advanced epithelioma,or between superficial bladder cancer and muscular layer-invasive bladder cancer(P0.05).The relative amount of DKK-1in bladder cancer tissues and adjacent tumor tissues was higher than that in normal tissues(2.033±0.372,1.816±0.352,and 0.976±0.202;P=0.018and 0.047).The protein expression of DKK-1in bladder cancer tissues was significantly higher than that in control group.And DKK1protein levels were elevated in the tissue surrounding blood vessels,but were not significantly increased in interstitial tissue.DKK-1positive rate in bladder cancer tissues was higher that that in normal tissue(48% vs.31%,P=0.020).Conclusion There is a close relationship between the incidence of bladder cancer and DKK1.DKK-1may become a new tumor marker for the detection and diagnosis of bladder cancer.
