摘要
目的探讨肠道病毒71型(EV71)重症手足口病患儿心电图及临床实验室预警指标的特征,为临床早期诊断、早期治疗提供依据。方法选择2011年5—10月唐山市妇幼保健院感染科收住院的重症手足口病患儿85例,根据病情分为轻症组50例,重症组35例。观察两组患儿心电图心率、心律失常及ST-T改变、血压、白细胞计数、空腹血糖、心肌酶的变化。结果与轻症组比较,重症组患儿心率加快,ST-T改变发生率增高,差异均有统计学意义(P<0.05);两组患儿房性期前收缩、室上性心动过速、室性期前收缩、室性心动过速、心室纤颤、室房传导阻滞发生率比较,差异均无统计学意义(P>0.05)。与轻症组比较,重症组收缩压、舒张压、白细胞计数、空腹血糖、肌酸激酶同工酶(CK-MB)水平升高,差异均有统计学意义(P<0.05);两组患儿天冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)、肌酸激酶(CK)水平比较,差异均无统计学意义(P>0.05)。结论重症手足口病患儿心率加快,ST-T改变发生率增高,心肌酶指标升高,心电图及实验室预警指标对临床早期筛查轻症及重症EV71手足口病患儿具有诊断价值。
Objective To investigate the clinical features of laboratory warning indexes and electrocardiogram in chil- dren with severe Hand - foot ：- mouth disease of type EV71 in order to provide reference for clinical early diagnosis and early treat- ment. Methods 85 children with hand - foot - mouth disease admitted to Tangshan Maternity and Child Healthcare Hospital from May to October 2011 were divided into mild type group （50 cases） and severe type group （35 cases） . The ECG heart rate, cardiac arrhythmia, ST- T changes, blood pressure, whilte blood cell count, fasting blood glucose and serum myocardial en- zymes of the two groups were observed. Results Compared with mild type group, the severe type group had significantly more rapid heart rate and higher ST - T abnormal rate （P 〈 0. 05 ） . The premature atrial contraction, SVT, VPC, VT, VF and atri- oventricular block incidence showed no statistically significant difference between the two groups （ P 〉 0. 05 ） . Compared with mild type group, the SBP, DBP, white blood cell count, fasting blood glucose and CK - MB were significantly increased （P 〈 0. 05） . The AST, LDH and CK between the two groups showed no statistically significant difference （ P 〉 0.05 ）. Conclusion Children with hand - foot - mouth disease have more rapid heart rate, higher ST - T abnormal changes rate, higher myocardial enzyme indexes. Features of electrocardiogram and laboratory warning indexes could be used as early screening of mild type or se- vere type in children with hand -foot -and -mouth disease.
