摘要
目的探讨不同程度冠状动脉病变高危人群肱-踝脉搏波传导速度(baPWV)、踝-臂指数(ABI)的差异及其与双源CT(DSCT)冠状动脉成像的相关性。方法选择高危对象138例,分别做DSCT冠状动脉成像、baPWV、ABI和血液等检查,按DSCT结果分组后,观察baPWV及ABI与冠状动脉病变的支数、最大狭窄程度、斑块性质和钙化总积分的关系。结果随冠状动脉病变支数和最大狭窄程度增多,baPWV(≥1400 cm/s)升高,ABI(<0.9)则减少;baPWV随软斑块-混合斑块-钙化斑块和钙化总积分增多呈逐级升高趋势,最高积分与最低积分比较,差异有统计学意义(P<0.05);钙化斑块组和混合斑块组的ABI分别较软斑块组和阴性组明显减少,最低组与最高组比较,差异有统计学意义(P<0.05)。结论 baPWV和ABI是反映动脉硬化较好的指标,可作为冠状动脉病变早期诊断、病变程度和危险分层的评估参数,联合DSCT可提高对冠状动脉斑块的性质、稳定性、易损斑块的识别,对防患急性冠状动脉综合征的风险有一定的意义。
Objective To investigate the differences of baPWV and ABI among high-risk population with different degrees of CAD,and to study the relationship between those parameters and DSCT Coronary Angiography in the Diagnosis of CAD. Methods 138 patients with high risk of CAD underwent DSCT coronary angiography. Tests of baPWV,ABI and blood examinations were done respectively. By observing the data set,which had been classified by the results of DSCT,the study explored the relationship between baPWV,ABI and the CAD counts,the maximum degree of stenosis,the nature of plaque and the total calcium score. Results The study found that as the number of the counts,the maximum degree of stenosis,and baPWV (i〉1400 cm/s)increase,ABI (〈0.9) decreases. A gradual rising trend was found in baPWV,with the increase of soft plague-mixed plague-calcific plaque and the total calcium score. The difference between the highest and the lowest group was significant (P〈0.05). ABI of calcific plague group and mixed plague group significantly decreases, compared to soft plague group and negative group. The difference between the highest and the lowest group was also significant. Conclusions baPWV and ABl are fair indicators of atherosclerosis,which can be used as evaluation parameters of severity and risk stratification,and also in CAD's early diagnosis. Combining DSCT is able to improve the identification accuracy of the nature, stability and vulnerability of plaque,and further to prevent the risk of ACS.
