ABI联合baPWV与双源CT冠状动脉成像的相关性研究 被引量:2

摘要 目的通过与双源CT对比,评价踝臂指数(ABI)、肱-踝脉搏波速度(ba PWV)在辅助预测冠状动脉病变的临床价值。方法搜集经双源CT冠状动脉造影(CTA)检查显示冠状动脉狭窄程度≥50%并行ABI及ba PWV检测的50例患者,分析ABI、ba PWV与CTA冠状动脉狭窄支数、节段以及斑块数目的相关性。结果单支、两支和三支病变组间的ABI值、ba PWV值存在差异(P<0.05),随着病变支数的增加,ABI逐渐减小,ba PWV逐渐增加。两支病变时,ABI均值<0.9;三支病变时,ba PWV均值>1400 cm/s,说明可能ba PWV值更敏感。ABI<0.9、ABI>0.9两组间检出斑块数无差异(P>0.05),狭窄节段数有统计学差异(P<0.02);ba PWV<1400 cm/s、ba PWV>1400 cm/s组间斑块数及狭窄节段有统计学差异(P<0.05)。表明ABI<0.9、ba PWV>1400 cm/s时冠状动脉狭窄可能性较大,ba PWV>1400 cm/s时冠状动脉斑块出现的可能性较大。结论 ABI和ba PWV与DSCT冠状动脉的狭窄支数、斑块数及狭窄节段数有关,可以用于辅助预测冠心病的严重程度。 Objective Using dual-source CT coronary angiography to evaluate the clinical application of ankle-brachial index( ABI),brachial ankle pulse wave velocity( ba PWV) in predicting coronary artery lesions.Methods A total of 50 patients with coronary artery stenosis( ≥50%),which was confirmed by dual-source CT coronary angiography,were enrolled in this study.Both ABI and ba PWV were determined in all the patients.The numbers of stenotic branches and segments as well as the numbers of atherosclerotic plaques were calculated,and their correlations with ABI and ba PWV were analyzed.Results Statistically significant differences in ABI and ba PWV values existed among single-branch,two-branch and three-branch lesion groups( P 〈 0.05).With the increasing of the number of diseased vessels,ABI value was decreased,while ba PWV value was gradually increased.In two-branch lesion group,ABI value was less than 0.9; in threebranch lesion group ba PWV value was larger than 1400 cm / s,indicating that ba PWV value might be more sensitive.The difference in the number of atherosclerotic plaques between ABI 〈 0.9 group and ABI 〉 0.9 group was not significant( P 〉0.05),while the difference in the number of stenotic segments between the two groups was statistically significant( P 〈 0.02).Statistically significant differences in the number of atherosclerotic plaques and in the number of stenotic segments existed between ba PWV value 〈 1400 cm / s group and ba PWV value 〉 1400 cm / s group( P 〈 0.05).The results indicated that coronary artery stenosis was most probably present when ABI was 〈 0.9 and ba PWV value was 〉 1400 cm / s,while coronary atherosclerotic plaque was most probably present when ba PWV value was 〉 1400 cm / s.Conclusion ABI and ba PWV values carry certain correlations with the number of coronary stenotic branches and segments as well as with the number of coronary atherosclerotic plaques.Therefore,it can be used in predicting the severity of coronary heart disease.
