摘要
目的 探讨多巴酚丁胺负荷试验 (DSE)中多普勒组织成像 (DTI)技术检测左室整体收缩功能变化的临床应用价值。方法 30例冠心病患者 ,1 8例正常人 ,按 5分钟递增法以 5、1 0、2 0、30μg/ (kg· m in)静注多巴酚丁胺 (DBA ) ,DTI记录注药前及每一剂量级时二尖瓣环运动情况。结果 收缩期二尖瓣环 6个位点平均速度 (Sa)正常组及单支病变组注药后升高显著 ,多支病变组注药后升高不显著。与正常组比较 ,单支病变组注药前、后均无显著差异 ;多支病变组注药前、后显著低于正常组。DTI判断 DSE前后左室收缩功能正常的敏感性、特异性和准确性分别为 86 .7%、88.9%、87.5 %。结论 使用 DTI测量二尖瓣环收缩期运动速度 ,可在负荷试验中评价左室整体收缩功能变化。
Objective To evaluate the value of Doppler tissue imaging (DTI) in assessment of left ventricular total systolic function in dobutamine (DBA) stress echocardiography (DSE).Methods Thirty patients with coronary artery diseases (CAD) and 18 normal subjects underwent DSE,with per 5 min adding dose of 5,10,20,30 μg/(kg·min)DBA.The mitral valve annulus velocities were recorded by DTI.Results Mean systolic velocities at 6 sites of mitral valve annulus (Sa) in single branch stenosis group were similar to those of normal group before and during DSE,while Sa rose up after DBA injection.Sa in the patients with two or more coronary stenosis was not increased significantly after DBA injection and had significant difference with those of normal group.△Sa>20% was selected as the standard of normal left ventricular systolic function,the sensitivity,specificity and accuracy of DTI to judge left ventricular systolic function abnormality were 86 7%,88 9%,87 5%.Conclusion Mitral valve annulus movement parameters measured by DTI can be used as criteria to evaluate left ventricular total systolic function changes.
