摘要
目的 :研究低分子量肝素 (low- molecular- weight heparin,L MWH)对人工晶体植入术后眼内纤维蛋白渗出的预防作用。方法 :110例老年性白内障随机分为 2组 ,并盲法给药。在进行白内障囊外摘除及后房人工晶体植入手术中 ,对照组灌注液为乳酸钠林格氏液 ,L MWH组于灌注液中加入 L MWH—速避凝 (fraxiparine) ,浓度为 6 u/ ml,。术中观察眼内出血 ,术后 1、 3及 7天观察眼内纤维蛋白渗出、房水混浊及眼内出血情况。结果 :L MWH组术后第 1、 3、 7天眼内纤维蛋白渗出及房水混浊轻于对照组(P<0 .0 5 ) ,两组术中及术后出血无显著性差异 (P>0 .1)。结论 :L MWH能有效抑制人工晶体植入术后眼内纤维蛋白渗出反应 ,并且不增加眼内出血危险性。
Objective:The preventive effects of a low molecular weight heparin (LMWH) on intraocular fibrin after intraocular lens implantation were studied.Methods:llo cases with senile cataract were randomly assigned into two groups.The surgery of extracapsular cataract extraction and intraocular lens (IOL) implantation was performed in a masked fashion on control eyes with lactated Ringers infusion and on LMWH eyes treated with fraxiparine 6u/ml in the infusate.The intraoperative bleeding was graded by surgeon.The amounts of fibrin and inflammatory cells and hemorrhage were graded by using slit lamp examination on the 1 st ,3 rd and7 th postoperative days.Results:The mean grads of fibrin and inflammatory cells in the LMWH eyes were lower than those in the control eyes (P<0 05).No statistically significant differences in the degree of intraoperative or postoperative hemorrhage were noted between the two groups.Conclusions:Low molecular weight heparin is an effective inhibitor of postoperative intraocular fibrin formation after extracapsular cataract extraction and IOL implantation without inducing ocular bleeding complications.
