氩氦刀冷冻术与微波消融术治疗肝癌的临床疗效对比研究 被引量:5

摘要 目的探讨氩氦刀冷冻术与微波消融术治疗肝癌近期疗效的对比分析。方法分析2011年1月~2013年1月120例肝癌患者,其中60例在CT引导下行经皮肝穿刺氩氦刀冷冻治疗,另外60例在CT引导下行经皮肝穿刺微波消融治疗,术后定期复查肿瘤标志物及CT或MRI随访。结果研究组小肝癌(≤3em)患者45例,84.4%(38/45)达到肿瘤完全坏死,15.6%(7/45)达到部分坏死;大肝癌(〉5em)患者15例,治疗后AFP降至正常范围或CT、MRI提示肿瘤完全坏死7例,占46.7%(7/15),8例治疗后AFP无明显变化或CT、MRI提示部分坏死占53.3%(8/15)。对照组小肝癌(≤3cm)患者45例,71.1%(32/45)达到完全坏死,28.9%(13/45)达到部分坏死;大肝癌(〉5em)患者15例,治疗后AFP降至正常范围或CT、MRI提示肿瘤完全坏死4例,占26.7%(4/15),11例治疗后AFP无明显变化或CT、MRI提示部分坏死占73.3%(11/15)。研究组与对照组对比分析(P〈0.05),差异有统计意义。结论CT引导下经皮肝穿刺氩氦刀冷冻消融术治疗肝癌是一种安全、有效的治疗方法。 Objective To analyze the curative effect of Argon-Helium cryoablation and microwave ablation therapy of primary hepatocellular carcinoma. Methods From January 2011 to January 2013, 120 patients with hepatocellular carcinoma were randomly divided into two groups. One group of 60 patients was treated with CT-guided percutaneous targeted Argon-Helium cryoablation and the second group of 60 patients was treated with CT- guided microwave ablation. Both groups were followed up by tumor markers and CT/MRI. Results In the Argon-Helium eryoablation group of 45 patients, 38 (84.4%) hepatocellular carcinomas measuring 3 cm or less showed complete and 7 (15.6%) partial necrosis. Of 15 liver tumors larger than 5 cm, 7 (46.7%) showed complete necrosis on CT/MRI with alpha fetoprotein returned to normal range whereas 8 (53.3%) showed partial necrosis on CT/MRI with no change in alpha fetoprotein level. In the microwave ablation group of 45 patients, 32 (71.1%) hepatocellular carcinomas measuring 3 cm or less showed complete necrosis and 13 (28.9%) showed partial necrosis. Of 15 patients with liver tumors larger than 5 cm, 4 (26.7%) showed complete necrosis on CT/MRI with alpha fetoprotein returned to normal range whereas 11 (73.3%) showed partial necrosis on CT/MRI with no change in alpha fetoprotein level. There was significant difference in treatment response between the two groups. Conclusions CT- guided percutaneous targeted Argon-Helium cryoablation therapy of liver cancer is safe and effective.
