摘要
目的探讨高剂量左氧氟沙星静脉注射液治疗常见下呼吸道感染的临床疗效。方法回顾性分析宁波市第二医院2010年5月至2012年6月收治的166例下呼吸道感染患者的临床资料,其中肺炎89例,慢性阻塞性肺炎加重期45例,支气管扩张症32例。所有患者均予滴注高剂量(600mg/d)左氧氟沙星注射液进行治疗,7~14d为1个疗程。观察其临床疗效及不良反应发生情况。结果166例患者发热、咳嗽、咳痰及肺部口罗音等临床症状平均缓解时间均不超过5.0d,三类患者WBC均下降(t=4.26、5.38、5.74,P均〈0.05),肝肾功能无损伤;平均治疗时间为7.9d,总有效率为83.7%。共检出细菌42株,治疗后36株被清除,清除率为85.7%。不良反应发生率为7.2%,均为轻度反应。结论静脉高剂量左氧氟沙星注射液治疗下呼吸道感染疗效明显,安全性高,患者不良反应发生率低,值得临床推广和应用。
Objective To evaluate the clinical efficacy of high-dose levofloxacin injection for treatment of lower respiratory tract infections, Methods The clinical data of 166 patients with lower respiratory tract infections received in the Second Hospital of Ningbo from May 2012 to June 2012 were retrospectively analyzed. Among these patients, there were 89 pneumonia cases, 45 chronic obstructive pulmonary disease (COPD) cases and 32 bronchiectasis cases. All patients were treated with high-dose levofloxacin (600 mg/d) for 7-14 days as one course of treatment. The clinical curative effect and adverse reaction were observed. Results The mean remission time of fever, cough, expectoration and pul- monary tales were less than 5 days in 166 cases. WBC decreased in all the pneumoina, COPD, bronchiectasis cases( t = 4.26,5.38,5.74, P 〈 0.05), no liver and kidney function damage was fotmd. The average treatment time was 7.9 days, and the effective rate was 83.7%. A total of 42 bacteria isolations were detected before treatment, and 36 bacteria isolations were cleared after treatment. The clearance rate was 85.7 %. The adverse reaction rate was 7.2 %, and all were mild. Condu^ons Intravenous injection of high-dose levofloxacin for treatment of lower respiratory tract infections is effective and safe with a low incidence of adverse reactions. It is worth of clinical generalization and applications.
