摘要
目的探讨23G玻璃体切除术(PPV)后高眼压发生率及相关危险因素。方法采用前瞻性研究方法将行PPV病人115眼纳入本研究。根据病人眼内不同操作分为PPV+眼内激光组、PPV+激光+冷凝组、PPV+激光+增殖膜剥除组及PPV+激光+白内障超声乳化组。根据晶状体情况分为有晶状体组和无晶状体组。视网膜脱离者常规行硅油或C3F8气体填充,并根据有无硅油填充分为有硅油组和无硅油组。以非接触眼压计测量术前、术后眼压,以眼压>2.973kPa作为高眼压标准。采用SPSS16.0统计软件进行分析。结果 40眼发生高眼压,其发生率为34.78%。不同眼内操作组间高眼压发生率比较,差异无显著性(P>0.05)。无晶状体组高眼压发生率高于有晶状体眼,差异有显著性(χ2=7.67,P<0.05)。有硅油组高眼压发生率与无硅油组比较,差异有显著性(χ2=7.92,P<0.05)。结论硅油填充为23GPPV后高眼压的危险因素,而不同眼内操作对PPV术后高眼压的发生无明显影响。
Objective To investigate the incidence and risk factors of high intraocular pressure （fOP） following 23-gauge pars plana vitrectomy （23G-PPV）. Methods A prospective study in 115 eyes scheduled for PPV was done. According to differ- ent intraocular procedures, the patients were divided into groups as group A： PPV combined with endolaser, group B： PPV＋ en- dolaser＋cryotherapy; group C： PPV＋endolaser＋retinal proliferative membrane peeling; and group D： PPV＋ endolaser＋ pha- coemulsification. According to lens status, there were two groups as phakic group and aphakic group. For retinal detachment eyes, filling with silicone oil or C3F8 was a routine, and based on whether or not filling with silicone oil, there were with-silicone group and without-silicone group, lOP was measured with a non-contact tonometer--before and after surgery. IOP〉2.973 kPa was taken as the standard of high IOP. Results High lOP occurred in 40 （34.78%） eyes. The incidence of high lOP between different pro- cedures was not statistically significant （P〉0.05）. The incidence of IOP in aphakic group was higher than phakic group （X^2=7.67, P〈0.05）. The difference of lOP incidence between silicon-filling and without-silicon-filling groups was significant （X^2= 7.92, P〈0.05）. Conclusion Silicone-oil tamponade is the risk factor of post-23G-PPV high IOP. Different intraocular procedures are not obviously related with high IOP after PPV.
