摘要
目的探讨经皮肾镜取石术(PCNL)术前置入输尿管支架管(D-J管),膀胱注水加压制造人工肾积水治疗无积水肾结石的有效性和可行性。方法无积水肾结石病人58例,术前患侧置入输尿管D-J管,尿道留置Foley三腔尿管,膀胱注水加压制造人工肾积水。超声引导下穿刺成功后开放尿管降低膀胱内压。结果 58例无积水肾结石病人均穿刺成功,取石顺利。结论 PCNL术前置入输尿管D-J管膀胱注水加压制造人工肾积水可降低肾穿刺难度,尤其适用于无积水肾结石者。
Objective To explore the efficacy and feasibility of creating an artificial nephrohydrosis before performing percutaneous nephrolithotomy （PCNL） for renal calculus in non-hydronephrotic kidney. Methods Preoperatively, a man-made nephrohydrosis was created by inserting a ureteral stent into the affected side ureter, the bladder was filled with water （through the indwelling three-way Foley urethral catheter） and compressed. Upon a successful puncturing under ultrasonic guidance, the cathe- ter was then opened to reduce the intravesical pressure. Results All the punctures were succeeded in 58 patients, and the PCNL carried out smoothly. Conclusion Artificial hydronephrosis may reduce the difficulty of renal puncturing, which is especially ap- plied to renal stones in kidney without hydronephrosis.
