不同亚型未服药注意缺陷多动障碍患儿的行为症状与父母压力的关系 被引量:7

Relationship between behavioral problems in different subtypes of untreated children with attention deficit hy-peractivity disorder and their parental stress

摘要 目的研究注意缺陷多动障碍(ADHD)患儿的行为问题和其父母压力的特点。方法使用Con—ners父母问卷(PSQ)和父母压力指数量表对88例不同亚型ADHD患儿进行症状及父母压力评估,并与31例健康对照儿童比较。结果ADHD患儿的PSQ各因子得分均高于健康对照组儿童,分别为品行(F=15.727,P=0.000)、学习问题(F=32.899,P=0.000)、冲动多动(F=33.761,P=0.000)和多动指数(F=28.394,P=0.000);混合型与注意缺陷为主型ADHD患儿在品行问题(t=2.226,P=0.029)、冲动多动(t=4.909,P=0.000)和多动指数(t=2.871,P=0.005)等因子上的差异均有统计学意义;ADHD患儿父母压力得分均高于健康对照组,并在儿童领域(F=3.848,P=0.011)、父母领域(F=2.879,P=0.039)以及压力总分(F=3.753,P=0.013)上的差异均有统计学意义;其中,混合型ADHD患儿的父母压力得分最高,心境指数的得分有统计学意义(t=2.231,P=0.028);相关性分析提示,父母压力总分与ADHD患儿的学习问题因子得分(r=0.224,P=0.011)和多动指数因子得分(r=0.177,P=0.045)呈正相关,且学习问题因子进入回归方程(t=2.588,P=0.011)。结论ADHD患儿比健康儿童表现出较突出的行为问题和父母压力,且父母压力与症状的严重程度呈正相关;混合型ADHD患儿存在更明显的父母压力,行为问题较多,值得关注。 Objective To investigate the behavior characteristics in different subtypes of untreated children with attention deficit hyperactivity disorder (ADHD) and their relationship with parental stress. Methods Eighty-eight children with ADHD and their parents were tested by Conners parents symptom questionnaire (PSQ) and Parenting Stress Index questionnaire ( PSI), and the results were compared among different ADHD subtypes, and 31 cases of healthy children were selected as healthy control group. Results Compared with the healthy controls,the ADHD children showed more behavior problems in conduct disorder subscale (F = 15. 727, P = 0.000 ) , learning problem subscale ( F = 32. 899 ,P = 0.000 ), inattention/hyperactive disorder subscale ( F = 33. 761, P = 0. 000 ) and hyperactive index subscale( F = 28. 394,P = 0.000) ; among the 3 ADHD subtypes, combined subtype had the highest scores, and there were significant differences in conduct disorder subscale ( t = 2. 226, P = 0. 029 ), inattention/hyperactive subscale ( t = 4. 909,P =0. 000) and hyperactive index subscale(t = 2. 871 ,P = 0. 005); compared with the healthy controls,the scores of PSI total stress ( F = 3. 753, P = 0. 013 ) , Parent Domain ( F = 2. 879, P = 0. 039 ) and Child Domain ( F = 3. 848, P = 0. 011 ) were significantly higher in the ADI-ID group, among those, combined subtype had the highest scores, and there was significant difference in the mood factor( t = 2.231 ,P = 0. 028 ). The PSI total stress scores were positively correlated with the scores of learning problem subscale (r=0. 224 ,P =0.011 )and the scores of hyperactive index subscale( r = 0. 177 ,P = 0. 045). At last the learning problem factor entered the regression equation ( t = 2. 588, P = 0. 011 ). Conclusions ADHD children have more behavior problems and parental stress, and parental stress is positively correlated with the severity of symptoms, especially in the combined subtype, which is particularly worthy of attentio
