右美托咪定对窒息型大鼠心肺复苏后心肌损伤的保护作用 被引量:7

摘要 目的观察大鼠心肺复苏(CPR)后左心室功能、血清心肌酶和心肌超微结构的变化,以及右美托咪定的干预作用。方法成年雄性SD大鼠60只,日龄70~95 d,体重290~360(327.5±32.4)g,采用随机数字表法,随机分为3组:假手术组(S组)、常规复苏组(CPR组)和右美托咪定组(DEX组),每组20只。S组仅行麻醉和动静脉穿刺置管。CRP组和DEX组建立窒息型大鼠心搏骤停-心肺复苏模型,DEX组于心搏骤停后即刻经颈外静脉注射右美托咪定50μg/kg,CPR组给予等量生理盐水。于窒息前(T0),自主循环恢复(ROSC)后15 min(T1)、30 min(T2)、60 min(T3)、120 min(T4)时观察左心室功能;于T4取动脉血,测定血清中肌酸激酶同工酶(CK-MB)和肌钙蛋白I(cTnI)含量,同时取左心室心肌标本,电镜下观察心肌超微结构,并行心肌细胞线粒体损伤评分。结果与T0比较,CPR组和DEX组ROSC后左心室功能明显降低(P<0.05),血清CK-MB和cTnI含量明显升高(P<0.05),心肌超微结构病理损害较重,线粒体评分明显升高(P<0.05)。与CPR组比较,DEX组ROSC后左心室功能明显升高(P<0.05),血清CK-MB和cTnI含量明显降低(P<0.05),心肌超微结构病理损害较轻,线粒体评分明显下降(P<0.05)。结论右美托咪定可改善窒息型大鼠CPR后左心室功能,减少血清CK-MB和cTnI的含量,减轻心肌细胞超微结构的损害,减轻大鼠心肌损伤,对心肌具有一定的保护作用。
