摘要
目的探讨奥氮平治疗对精神分裂症患者胃动力学的影响。方法将40例未经治疗的精神分裂症患者随机分为奥氮平组与阿立哌唑组,每组各20例,分别接受奥氮平和阿立哌唑治疗6周,于治疗前及治疗第6周末,测定两组的胃窦蠕动幅度、胃窦收缩频率、胃排空时间、体质量及瘦素水平。结果①治疗第6周末,奥氮平组胃窦蠕动幅度、胃窦收缩频率、胃排空时间、体质量、瘦素与治疗前比较,差异有统计学意义(P<0.05),而阿立哌唑组上述指标治疗前后比较,差异无统计学意义;②奥氮平组治疗前后胃排空时间变化值与体质量变化值(r=-0.501,P=0.034)、瘦素变化值(r=-0.507,P=0.022)呈负相关,体质量变化值与瘦素变化值(r=0.601,P=0.02)呈正相关。结论精神分裂症患者接受奥氮平治疗后胃排空加快,并与其体质量的增加、瘦素的增高有关。
Objective To explore the influence of olanzapine in the treatment of schizophrenic patients on gastric motility.Methods Untreated schizophrenic patients treated with olanzapine（n=20） and treated with aripiprazole（n=20）were involved in the study.Gastric contraction amplitude,gastric contraction rate,gastric emptying time,body weight and leptin were measured at baseline and at 6 weekend.Results ① Gastric contraction amplitude was（3±0.8）cm2,gastric contraction rate was（3.2±0.7）min,gastric emptying time was（52.6±7.4）min,body weight was（57.3±6.2）kg,leptin was（5.1±2.8）ng/l in olanzipine group at 6 weekend changed significantly compared to those before the treatment[（2.1±0.6）cm2,（2.8±0.8）/min,（56.9±8.3） min,（54.1±6.7） kg,（3.1±2.2）ng/l]（P=0.000,0.000,0.000,0.000,0.01）;Those differences in aripiprazole group were not significant.②In olanzapine group,the gastric emptying time changed was negatively correlated with the body weight changed（r=-0.501,P=0.034）and leptin changed（r=-0.507,P=0.022）,the body weight changed was positively correlated with the leptin changed（r=0.601,P=0.02）Conclusion The data indicates that the gastric empy.ting rate quiken after olanzapine treatment in the schizophrenic patients.Moreover,the quikening of gastric emptyting rate have relationships with that of body weight increased and leptin level elevated.
