摘要
目的研究奥扎格雷联合法舒地尔对短暂性脑缺血发作(TIA)患者的脑血流及Lp-PLA2活性、NSE、炎性因子的影响。方法该院2010—2012年间收治的100例TIA患者随机分为实验组和对照组各50例,另选取同期来院体检的50例健康人作为健康组,三组受检者均接受颅多普勒(TCD)检查及测定Lp-PLA2、NSE、TNF-α、hs-CRP水平等。对照组口服阿司匹林治疗,实验组在对照组基础上接受奥扎格雷联合法舒地尔治疗,比较两组治疗前后及与健康组差异。结果治疗前实验组和对照组患者大脑前动脉、中动脉、后动脉平均流速均显著低于相应健康组(P<0.05),NSE、Lp-PLA2、TNF-α和hs-CRP水平均显著高于健康组(P<0.05)。经治疗后两组患者各动脉血流均较治疗前有显著提高(P<0.05),NSE、Lp-PLA2、TNF-α和hs-CRP水平均显著下降(P<0.05),但实验组动脉平均流速高均于对照组(P<0.05),而NSE、Lp-PLA2、TNF-α和hs-CRP水平均低于对照组(P<0.05)。结论发生TIA时颅内动脉血液流速明显降低,而NSE、Lp-PLA2、TNF-α和hs-CRP水平显著升高。使用奥扎格雷联合法舒地尔对TIA患者能显著改善脑血流,恢复动脉流速,降低NSE、Lp-PLA2、TNF-α和hs-CRP,对TIA有较好的治疗效果。
Objective To study the effect of ozagrel combined with fasudil on transient ischemia attack in terms of cerebral blood flow,Lp-PLA2,NSE and inflammatory factors.Methods The 100 TIA patients during 2010—2012 were randomly divided into experimental and control group,and 50 healthy people were selected as healthy group.The control group was treated with oral aspirin while the experimental group had ozagrel combined with fasudil.The cerebral blood flow and Lp-PLA2,NSE,TNF-α,hs-CRP level were observed.Results The TIA patients had significantly lower velocity of ACA,MCA and PCA and higher level of Lp-PLA2,NSE,TNF-α and hs-CRP compared with healthy group（P 0.05）.The experimental group and control group both increased in terms of blood flow velocity and decreased in terms of Lp-PLA2,NSE,TNF-α and hs-CRP（P 0.05）.But the experimental group had higher velocity and lower Lp-PLA2,NSE,TNF-α and hs-CRP level compared with control group（P 0.05）.Conclusion The blood flow velocity decreases and Lp-PLA2,NSE,TNF-α and hs-CRP level increase when TIA develops.The ozagrel combined with fasudil expands the vessels and improves the blood flow and lowers the Lp-PLA2,NSE,TNF-α and hs-CRP level,which has good effect on TIA.
