摘要
目的探讨三维适形放射治疗(3DCRT)联合顺铂(DDP)和5氟尿嘧啶(5-Fu)的(DF)方案化疗治疗食管癌的临床疗效。方法将收治的67例食管癌患者随机分为治疗组(32例)和对照组(35例),对照组患者给予3DCRT放射治疗,治疗组患者在3DCRT放射治疗基础上加用DF方案化疗。比较两组患者近期疗效,1年、2年和3年生存率,治疗完成后患者卡氏评分情况及不良反应情况。结果治疗组和对照组患者1年生存率(82.9%和59.4%)、2年生存率(45.7%和34.4%)和3年生存率(37.1%和25.0%)比较,差异均有统计学上意义(P<0.05)。治疗组患者KPS评分提高、稳定、降低分别为45.7%、51.4%和2.9%,对照组患者分别为31.3%、56.3%和12.5%,两组患者提高和降低间差异有统计学意义(P<0.05)。治疗组患者不良反应发生率为60.0%,对照组为43.8%,两组比较,差异无统计学意义(2=1.15 P>0.05)。结论 3DCRT联合DF方案化疗治疗食管癌疗效确切,提高了患者1年、2年、3年生存率及生活质量,且不良反应发生率不增加。
Objective To analyze the curative effect of three dimensional conformal radiation therapy combined with chemotherapy of DF regimen in the treatment of esophageal carcinoma.Methods We analyzed 67 cases who had treated esophagus cancer from January 2007 to January 2009,the 67 cases were randomly divided into two groups of 35 cases in the treatment group who had given 3DCRT and DF project,32 case in control group who had given 3DCRT.The effective,one year,two years,three years of survival rate,The quality of life,adverse effect were compared.Results The one year survival rate was 82.9% and 59.4%,two years survival rate was 45.7% and 34.4%,three years survival rate was 37.1% and 25.0%,the differences were statisticly significant（P0.05）.The KPS score level of improve,stability,lower were 45.7%,51.4% and 2.9% in the treatment group,31.3%,56.3% and 12.5% in the control group,and there were significant statistics difference（P0.05）.The incidence of adverse reaction in the treatment group was 60.0%,43.8% in the control group,no difference was found between two groups（2=1.15,P0.05）.Conclusions The 3DCRT combined with DF regimen were effectively,improve in one,two,three years of survival rate and the quality of life,the incidence of adverse reaction does not increase.
