子痫前期患者胎盘蜕膜组织CD14^+巨噬细胞的表达及意义 被引量:2

Significance of CD14^+ macrophages expression in placenta decidual tissue for patients with preeclampsia

摘要 目的探讨子痫前期患者胎盘局部蜕膜组织CD14+巨噬细胞表达水平与子痫前期的发病关系。方法选取2010年1月至2012年6月在中国医科大学附属盛京医院住院分娩的重度子痫前期患者30例为实验组,年龄(28.94±3.98)岁,怀孕(36.56±2.94)周。选择同期分娩的正常妊娠妇女30例作为对照组,年龄(27.50±3.61)岁,孕(38.97±3.03)周。均排除内科合并症和其他产科并发症。采用免疫酶化学技术分析两组蜕膜组织CD14+巨噬细胞水平变化,同时对蜕膜CD14+巨噬细胞水平与产妇平均动脉压、24h尿蛋白定量以及新生儿体质量的Spearman秩相关性分析。结果实验组患者CD14+巨噬细胞水平表达阳性率为85.56%,明显高于对照组的45.22%,差异有统计学意义(χ2=10.335,P=0.001);CD14+巨噬细胞水平与产妇平均动脉压、24h尿蛋白定量呈正相关(r=0.582、0.453,P<0.05),与新生儿体质量呈负相关(r=-0.108,P<0.05)。结论胎盘组织中蜕膜CD14+巨噬细胞水平与子痫前期的发病有关,且与病情的严重程度相关。 Objective To explore the levels of CD14 + macrophages expression in partial placenta decidual tissue for preeclampsia patients, and the relationship with the incidence of preeclampsia. Methods Thirty pregnant patients with severe preeelampsia in Shengjing Hospital of China Medical University were choosen as experimental group between January 2010 and June 2012. Mean age of the patients was (28.94± 3.98 ) years, and pregnancy ( 36.56± 2.94) weeks. Thirty cases of normal pregnant women hospitalized at the same time were choosen as control group, aged (27.50±3.61 ) years, pregnancy (38.97± 3.03) weeks. The two groups were excluded from the internal medicine complications and other obstetric complications. CD14+ levels in decidual tissue macrophages of the two groups were analized by immune enzyme cytochemistry technique. At the same time, the correlations of decidual macrophage CD14 +, maternal mean arterial pressure, 24 hours urine protein and neonatal weight were analized using Spearman rank correlation. Results Positive CD14 + macrophages expression of the experimental group patients was 85.56%, which was higher than 45.22% that in the control group, and the difference was statistically significant ( P = 0. 001, X2 = 10. 335 ). The level of CD14+ macrophages was positively correlated with maternal mean arterial pressure ( r = 0. 582, P 〈 0.05 ) and 24 hours urine protein ( r = 0. 453, P 〈 0.05 ), and negatively correlated with neonatal birth weight ( r = - 0. 108, P 〈 0.05 ) respec- tively. Conclusions The levels of CD14+ maerophages in placenta decidual associated with the pathogenesis and the severity of preeclampsia.
