摘要
目的初步探讨肝门右切迹参照法在腹腔镜胆囊切除术(LC)中的临床应用及其优点。方法回顾性分析389例LC术中采用肝门右切迹参照法和375例未采用肝门右切迹参照法在手术时间、术中出血量、中转开腹率、胆道损伤及住院时间等方面的差异;同样方法进一步分析比较151例有肝门右切迹患者且采用肝门右切迹参照法和151例无切迹患者临床观察指标的差异。结果采用肝门右切迹参照法组和未采用肝门右切迹参照法组之间比较,前者手术时间短、术中出血量少,中转开腹率低,两组之间差异有统计学意义,住院时间无明显差异,胆道损伤发生例数虽不符合统计学处理条件,但患者是受益的,另外有切迹且采用肝门右切迹参照法组和无切迹组之间比较,也得出同样的结果。结论在LC术过程中应用肝门右切迹参照法,具有手术时间短、术中出血量少、中转开腹率低、胆道损伤少等优点,值得在临床实践中进一步探讨。
Objective To preliminary explore of the clinical application and its advantages of porta right incisure reference method in laparoseopie cholecystectomy （LC）. Methods A retrospective analysis of 389 cases of LC intraoperative porta fight incisure reference method and 375 cases without porta fight incisure reference method is adopted in operation time, blood loss, conversion to laparotomy rate, biliary tract injury and hospitalization time; Same way further comparative analysis the different clinical observation index between group of 151 cases of patients with porta fight incisure and group of 151 patients without porta fight incisure. Results Porta fight ineisure method without using group and porta right reference incisure reference method comparison between groups, the former are shorter operation time, less blood loss, low conversion to laparotomy rate, and they were statistically significant difference between the two groups. No difference be- tween hospitalization time, although cases biliary tract injury is not in conformity with the statistical processing conditions, but the patients are all benefitted. Other groups are also obtained the same result. Conclusion In the process of LC of application Porta fight incisure refer- ence method, with shorter operation time, less blood loss, less conversion to laparotomy rate, and less biliary injury etc. So it is worthy of further study in clinical practice.
