摘要
目的:探讨严重牙周病拔牙后即刻种植覆盖义齿的临床效果。方法:选择2010年3月-2012年10月笔者所在医院收治的56例严重牙周病患者,种植前给予相应的牙周治疗,口腔卫生状况得到良好控制后拔牙即刻种植并覆盖义齿修复,对种植体的临床效果进行评价。结果:56例患者共植入120颗种植体,术后随访2年,51例患者取得良好的稳定性和满意度,X线检查结果显示种植体周围骨质相对稳定,骨垂直吸收速率平均都在1mm/年以下,5年吸收平均不超过2mm,临床效果评价均满意,其中9例患者术后2年内做过义齿的重衬或修补。另外5例患者17颗种植体因牙周健康状况控制不理想在术后2年内相继松动而失败。结论:即刻种植作为严重牙周病患者引起牙列缺失的主要治疗手段,术前只要牙周状况得到良好控制,严格要求手术操作,术后对牙周进行良好的维护,可达到满意的临床效果。
Objective： To explore the clinical effect of overdenture by immediate implant after tooth extraction due to severe periodontal diseases. Methods ： 56 patients with severe periodontal disease admitted to in our hospital from March 2010 to October 2012 were selected.After corresponding periodontal treatment and oral hygiene control, 56 patients treated with tooth extraction and then received overdenture by immediate implant.The effect was evaluated.Results： 287 implants were implanted in 56 patients.Good stability and satisfaction were achieved in 51 patients.X-ray examination results showed that the bone around the implant was relatively stable and vertical bone absorption rate was less than 1 mm/year, 5-years average absorption being not more than 2 mm.Among them, 9 patients were treated with denture reline or repair 2 years after surgery.The rest of 5 patients with 17 implants control was not satisfactory due to planting failure.Conclusion： As a main therapy for patients with severe periodontal disease, immediate implant can achieve satisfactory clinical results so long as preoperative periodontal status are well controlled and operation is performed strictly.
