摘要
目的探讨即刻种植覆盖义齿在牙周病患者牙列缺失修复中的临床效果,为牙周病患者失牙后的即刻种植修复提供临床依据。方法回顾性分析我院1999-2009近10年期间完成的牙周病患者即刻种植覆盖义齿修复的临床资料65例,结合近年来口腔种植学的基础研究,分析其中成败的主要影响因素和可能规律。结果一方面即刻种植显著缩短了常规种植的疗程,为患者减少了牙列缺失所造成的咀嚼、发音或面容等不适,得到患者较高的认知度和满意度;另一方面,即刻种植也有效减缓和减少了牙槽骨的萎缩吸收,能更好地利用余留颌骨的有限条件达到较为稳定的种植效果,为长期稳定的修复效果提供了有力保障。其疗效受多因素影响,其中牙周健康状况的控制和合理的机械力学分布至关重要,二者相互影响,共同制约最终疗效。结论即刻种植作为牙周病所导致的牙列缺失的重要治疗手段之一,将是今后临床研究的主要方向。种植时机的选择和覆盖义齿修复方案的设计是关键所在。
Objective To investigate the treatment effects of overdenture supported by immediate implant in periodonaopathy.Methods A retrospective analysis was made on all the clinical data about 65 patients treated with overdenture by immediate implant with periodongtal disease during 10 years from 1999 to 2009 so as to analyze the factors affecting treatments and obtain possible regularity.Results Immediate implant won recognition of the patients by greatly reducing the course of treatment of conventional implant and indispositions.On the other hand,it reduced the analosis of alveolar bone so as to ensure a stable recovery.The effects were conditoned by many factors,of which the periodontal health status and rational distribution of mechanical force are of great importance.Conclusion Overdenture by immediate implant may be one of the most important effective treatments for patients with periodontal disease.The key points are the implant timing and recovery plan for overdenture.
