The prognostic value of ankle-brachial index in patients with ischemic stroke

摘要 目的探讨ABI与心脑血管事件发生的相关性,评估ABI在缺血性脑卒中患者预后判断中的临床意义。方法测量缺血性脑卒中患者的踝肱指数(ankle-brachial index,ABI)及随访18个月,选取缺血性脑卒中住院患者共107例,采用四肢同步自动测量动脉硬化检测仪测量所有患者的ABI,对入选患者进行随访,观察患者出院18个月内,主要心脑血管事件的发生情况,分析ABI与心脑血管事件之间的关系。结果107例缺血性脑卒中患者中,正常ABI组62例,低ABI组45例,随访结果发现,与正常ABI组相比较,低ABI组心脑血管事件发生率明显增高(33.3%vs.12.9%,P=0.011),多因素Logistic回归分析显示,在校正了其它相关危险因子后,低ABI是缺血性脑卒中患者发生心脑血管事件的独立预测因子(P=0.015)。结论低ABI对缺血性脑卒中患者的预后有临床价值,低ABI的缺血性脑卒中患者心脑血管事件发生率高,ABI可以作为缺血性脑卒中患者预后判断的指标。 Objectives To investigate the correlation between ankle-brachial index(ABI) and cardiovascular and cerebrovascular events, and explore the clinical predictive value of ABI for further vascular events in patients with isch-emic stroke, and provide clinical evidence for secondary prevention and treatment in patients with ischemic stroke, through repeated ABI measurements in patients with ischemic stroke during 18 months following onse of stroke. Meth-ods A total of 107 patients with ischemic stroke were enrolled. A synchronous measurement device of four limbs" s ath-erosclerosis was used for ABI measurement. All patients were followed up over an 18-month period for observing the main cardiovascular and cerebrovascular events. Relationships between ABI and cardiovascular and cerebrovascular events were analyzed. Results Among 107 stroke patients, 62 patients had normal ABI whereas 45 patients had low ABI. Compare with normal ABI group, the low ABI group had higher incidence of cardiovascular and cerebrovascular events (33.3 % vs. 12.9 %, P=-0.011). Low ABI was independent prediction factor of main cardiovascular and cerebrovascular events after adjustment of other vascular risk factors in logistic regression analysis (P=-0.O15). Conclusions The morbidity of cardiovascular and cerebrovascular events is higher in stroke patients with low ABI than in those with normal ABI, sug-gesting a predictive value of low ABI for further vascular events in patients with ischemic stroke. Thus, ABI can be used as prognostic indicator of patients with ischemic stroke.
